In consideration of being allowed to participate in the player tryouts organized by WEBS and Akron Sports, Inc., I, the undersigned, on behalf of myself, my heirs, executors, administrators, and assigns, hereby acknowledge and agree to the following:

Team Placement & Formation Policy: Participation in tryouts does not guarantee placement on a team. Team selections will be based on evaluations conducted by coaches and staff to ensure competitive balance and appropriate player development.

If registration numbers allow, additional teams may be formed; however, this will occur only if there are enough qualified players and available coaches to field a competitive team. The organization reserves the right to determine the number of teams based on player participation, coaching availability, and overall program needs.

Assumption of Risk: I understand that participation in player tryouts involves inherent risks, including but not limited to physical injury, illness, property damage, and the risk of contracting contagious diseases. I voluntarily and knowingly assume all such risks associated with my participation.
Release and Waiver: I, for myself and my heirs, hereby release, discharge, and hold harmless WEBS and Akron Sports, Inc., its officers, directors, coaches, volunteers, and all associated personnel, from any and all claims, demands, actions, or causes of action, whether for personal injury, property damage, or otherwise, arising out of or in connection with my participation in the player tryouts, including any actions or negligence on the part of WEBS and Akron Sports, Inc. and its personnel.
Medical Treatment Authorization: I authorize WEBS and Akron Sports, Inc. and its representatives to seek and obtain any necessary medical treatment or attention in the event of an injury, illness, or medical emergency during the tryout. I understand that [Organization Name] will make reasonable efforts to contact me or my emergency contact before seeking medical treatment, but they may proceed with necessary medical care if immediate attention is required.
Compliance with Rules and Guidelines: I agree to comply with all rules, guidelines, and instructions provided by WEBS and Akron Sports, Inc.during the player tryouts.
Photography and Publicity Release: I grant WEBS and Akron Sports, Inc.the right to use photographs, videos, or other likenesses of me taken during the tryouts for promotional, educational, and archival purposes.
Parent/Guardian Consent: If the participant is a minor, the parent or legal guardian must consent to this waiver and release of liability on their behalf.
I have read and understand this Waiver and Release of Liability, and I voluntarily sign it as my own free act and deed. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against WEBS and Akron Sports, Inc..
