As the parent or legal guardian of the participant(s) named below, I give permission for my child(ren) to participate in the Pilgrim Home Baptist Church (PHBC) water slide and related activities. I understand that participation in water activities and inflatable/water-slide attractions involves inherent risks, including, but not limited to, slipping, falling, collisions, drowning, and other injuries that may occur before, during, or after the activity.
I voluntarily assume all risks associated with my child(ren)’s participation and agree not to hold PHBC, its pastors, staff, volunteers, representatives, or affiliated organizations liable for any injury, illness, loss, or damages arising from participation in these activities.
By signing this agreement, I release and hold harmless PHBC, its leadership, staff, and volunteers from any and all claims, liabilities, demands, actions, or financial responsibility related to my child(ren)’s participation, regardless of the cause or circumstances.
I certify that my child(ren) is physically able to participate in these activities and that I will not hold PHBC responsible for injuries resulting from pre-existing medical conditions, failure to follow safety instructions, or misuse of equipment or attractions.
I understand that all participants are expected to follow safety instructions and directions given by PHBC staff and volunteers. I agree to encourage my child(ren) to follow all rules and safety guidelines during the event.
In the event of an emergency, I authorize PHBC staff or volunteers to obtain emergency medical treatment for my child(ren) if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred as a result of such treatment.
