ASSUMPTION OF RISK, RELEASE, LIABILITY WAIVER, INDEMNITY AGREEMENT, AND PRIVACY POLICY

PART 1: ASSUMPTION OF RISK & RELEASE

I, {name}, hereby acknowledge and understand the physical demands and potential risks associated with practicing martial arts, including but not limited to activities such as punching, kicking, striking, and grappling with both individuals and partners. In consideration of being allowed to participate in the 2026 Lepercq Memorial, I voluntarily assume all risks and responsibilities for any injuries or damages that may arise from my participation.

I understand that I am solely responsible for monitoring my own activity level and will immediately discontinue participation in any activity that I perceive to be unsafe or beyond my physical capabilities. I further acknowledge that the 2026 Lepercq Memorial, its administrators, members, directors, agents, officers, volunteers, employees, and other participants, as well as the owners and lessors of the premises where the activity takes place, shall not be held liable for any injuries, claims, demands, losses, or damages, whether caused by my own actions or inactions, the actions or inactions of others, or the conditions of the facility.

I hereby release, discharge, and covenant not to sue the 2026 Lepercq Memorial, its administrators, members, directors, agents, officers, volunteers, employees, and other participants, as well as the owners and lessors of the premises, from any and all liability arising from my participation in the event. I agree to indemnify and hold them harmless from any claims, demands, losses, or damages, including legal fees, incurred as a result of my participation.

I affirm that I am in proper physical health and condition to participate in the 2026 Lepercq Memorial. I understand that disruptive behavior may result in my exclusion from the tournament at the discretion of the organizers. Furthermore, I grant consent for any pictures or videos taken of me during the event to be used by ShotoCup for publicity or promotional purposes without any expectation of compensation.

PART 2: PRIVACY POLICY

In connection with my participation in the 2026 Lepercq Memorial, I acknowledge and agree to the following privacy terms:

Information Collected: The event organizers, ShotoCup, and their authorized agents may collect personal information from me, including but not limited to my full name, contact information (email, phone number, address), date of birth, emergency contact details, medical information disclosed for safety purposes, signature, and any photographs or video recordings captured during the event.

Purpose of Collection: My personal information will be used solely for the following purposes:

• Registration and check-in at the event
• Emergency contact and medical response coordination
• Communication regarding event updates, schedule changes, or safety notices
• Publicity, promotional materials, and archival documentation (specifically for photographs and video, as consented to in Part 1 above)
• Compliance with any legal or insurance requirements

Data Sharing: My personal information will not be sold, rented, or traded to third parties for marketing purposes. It may be shared only with:

• Event staff, volunteers, and medical personnel on a need-to-know basis
• Legal or insurance authorities as required by applicable law
• Owners or lessors of the premises where required for liability or safety reasons
• Data Retention: My personal information will be retained for no longer than is necessary to fulfill the purposes stated above, or as required by law.
• Photographs and video may be retained indefinitely for promotional and archival purposes by ShotoCup.

Data Security: Reasonable physical, electronic, and procedural safeguards will be maintained to protect my personal information from unauthorized access, disclosure, or misuse. However, I acknowledge that no data transmission or storage system is 100% secure.

My Rights: I understand that I may request access to, correction of, or deletion of my personal information by contacting ShotoCup in writing at the address provided during registration. I also understand that withdrawal of consent for use of my images or video may not apply retroactively to materials already published.

Acknowledgment: By signing this agreement, I confirm that I have read and understood this Privacy Policy and consent to the collection, use, and disclosure of my personal information as described above.

PART 3: SIGNATURE

By signing this agreement, I acknowledge that I have carefully read and fully understand the entire document, including both the liability waiver and the Privacy Policy, and voluntarily enter into this release, indemnity, and privacy agreement.
