Every camper needs:

• Gym shoes
• A sack lunch

Weather Policy: If inclement weather forces a cancellation, we will notify you via email and/or text by 8:45 AM, June 30. The camp will not be rescheduled.

Informed Consent and Acknowledgement

I acknowledge that participation in sports activities including basketball, soccer, baseball, and softball involves inherent risks of injury. I voluntarily allow my child to participate in Haven Church Summer Fun Sports Camp and assume those risks on behalf of my child.

I release and hold harmless Haven Church, its staff, volunteers, and representatives from liability for injuries or damages arising from participation in camp activities, except in cases of gross negligence or intentional misconduct.

Medical Release and Authorization

As the parent and/or legal guardian of the named child(ren), I authorize Haven Church staff and volunteers to obtain emergency medical treatment for my child if I cannot be reached immediately.

I authorize qualified and licensed medical professionals to provide any necessary emergency medical care, including examination, treatment, x-rays, and minor medical procedures deemed necessary for my child’s health and safety.

In the event of a serious illness or injury, every reasonable effort will be made to contact me as quickly as possible before further treatment is provided.

I also authorize Haven Church, its directors, staff, and volunteers to provide basic first aid and emergency care prior to my child’s admission to a medical facility.

This authorization is granted voluntarily for the purpose of protecting the health and safety of my child during participation in Haven Church Summer Fun Sports Camp.
