Foster Application Form
  • Foster Application Form

Apply to become a foster caregiver for animals in need.
  • Format: (000) 000-0000.
  • Do you rent or own your home?*
  • If you rent, does your landlord allow you to have another pet?*
  • Do we have permission to contact your landlord?*
  • Do you currently have pets?*
  • Have you fostered animals before?*
  • What types of animals are you interested in fostering?*
  • Do you have a fenced yard?*
  • I confirm my fence has been inspected for escape risks*
  • Are you willing to . . . ?*
  • If you are fostering a cat/kitten, do you agree to keep it indoors only and not let it outside?*
  • Do you understand that it can take days, months, even years to get a foster animal adopted? Please indicate if you can commit until the animal is adopted. If requested in writing, you will return the foster animal to the rescue within 12 hours of the request. This is at the discretion of Dr. Wolley and can be made at anytime for any reason. Failure to do so will be considered a theft and the sheriff's office will be contacted to recover the rescue animal. In the state of California, animals are considered property. By checking "yes" you also agree to this term.*
  • Our foster animals need to get to veterinary appointments for vaccines/dewormers, and medical treatment. Can you commit to making sure your foster animal will be at any veterinary appointments needed?*
  • The best way to get your foster animal adopted is to bring them to adoption events. Are you willing to attend adoption events at the very least once a month?*
  • Our foster homes follow up with potential adopters with approved applications. Do you agree to make your foster animal available for meet and greets with potential adopters?*
  • If you cannot keep your foster animal for any reason do you agree to return the animal to us and provide a reasonable amount of time to find a new foster?*
  • Do you acknowledge that we require preauthorization for ANY veterinary care for a foster dog and that IF you don't get Preauthorization YOU will be responsible for payment?*
  • Do you agree that Junebug Foundation makes the FINAL determination of adoption for any foster animal and if you find a potential adopter Junebug Foundation will make the determination if the adopters are suitable?*
  • Do you understand that if you decide to adopt your foster animal, you will need to go through a separate adoption process? Fostering an animal does not guarantee that you will be its forever home.*
  • Even the best trained and socialized animals pose a risk of damage to property or injury to people or other animals. In addition, the process that leads a rescue pet to needing a foster home is inherently stressful and traumatic. In the short time that we observe our rescues before placing them with fosters, we have no way of evaluating their dispositions and habits. By checking this box, you agree that you understand that fostering a rescue pet comes with the inherent risk of personal injury (including, for example, scratching and biting injuries) and property damage (including, for example, chewing on items or urination / defecation in the house), and you expressly assume that risk as a condition of taking part as a Junebug foster.*
