  • This agreement is between Wolfe Pack Dog Training, LLC, and the dog owner submitting this agreement form, effective as of {submissionDate}, until cancelled.


    Scope of Services

    Descriptions of services are summaries and may not include every activity performed as part of the service.

    The services that this agreement applies to are:

    • In-home training, consisting of basic obedience including sit, down, stay, recall, threshold training, structured walking, and the like
    • Group training classes conducted at a local park or public facility in a group setting that includes other dogs
    • "Board and train" style training of the dog which is conducted while the dog is under housing and care by the trainer
    • Boarding, which includes housing and care of the dog; new training is not conducted, but any existing training will be reinforced during the dog's stay
    • Daycare, which consists of the dog owner dropping the dog off for a "play group" session and socialization with other well-mannered dogs, plus one short individual training session focused on your dog

    If you choose to utilize additional services in the future, this agreement applies to those services as well, and this agreement need not be resubmitted.

    However, if you are enrolling multiple dogs in our services, please submit this form once for each dog so that we have complete records.

    Owner Responsibility

    It is the dog owner's responsibility to continue training independently outside of professional training sessions or other services to help the dog retain the training and prevent regression of behavior.

    The dog owner understands that participation in dog training, daycare, boarding, and related activities involves inherent risks, and agrees to assume those risks.

    Dogs are animals, and behavior can never be guaranteed. Training results vary based on owner's consistency with application and reinforcement of training. Wolfe Pack Dog Training cannot guarantee elimination of unwanted behaviors.

    Communication Expectations

    Non‑emergency communication will be responded to during normal business hours.

    Client Information Confidentiality

    Client information provided in this form is used solely for the purpose of providing training and care services.

    Right to Photograph for Safety Documentation

    Wolfe Pack Dog Training, LLC may photograph or video record training sessions for safety documentation, record‑keeping, or training review purposes. The owner's media release choice will be honored for social media purposes.

    Refund Policy

    Wolfe Pack Dog Training has a "no refund" policy and is not liable for behavior of the dog following conclusion of the service, except where otherwise stated in this Agreement.

    Eligibility

    Your dog's behavior and temperament will be evaluated to determine eligibility for training. Any known potentially dangerous behavior(s) that may cause the dog to be considered a dangerous dog must be disclosed in this form submission and further discussed and evaluated during the initial consultation. Such potentially dangerous behavior includes but is not limited to: aggression toward dogs or other animals, aggression toward humans, resource guarding, or destructive behavior.

    "Dangerous dog" is defined by Florida Statute 767.11(3) as a dog which has aggressively bitten, attacked, or endangered or has inflicted severe injury on a human being on public or private property; has more than once severely injured or killed a domestic animal while off the owner's property; or has, when unprovoked, chased or approached a person upon the streets, sidewalks, or any public grounds in a menacing fashion or apparent attitude of attack, provided that such actions are attested to in a sworn statement by one or more persons and dutifully investigated by the appropriate authority.

    Dogs must be current on their rabies vaccination and proof must be furnished. If the dog is not current on the rabies vaccine, they are ineligible to be considered for services.

    Should the dog's behavior be discovered to not meet eligibility requirements, Wolfe Pack Dog Training reserves the right to terminate future training of the dog. Refunds will be considered at the trainer's discretion in this circumstance.

    Failure to disclose known behavioral issues may result in immediate termination of services without refund.

    Safety

    All dogs shall utilize a "safety clip" which secures the dog's collar to any other corrective collar equipment in use during services provided. The dog owner agrees to use equipment recommended by the trainer and is responsible for ensuring that all collars, leashes, and other gear are in safe working condition.

    Should any newly noticed aggressive or reactive behavior toward other animals or humans be noticed by the dog owner, this must be disclosed before the next training session.

    During group class services, the dog owner must clean up any waste generated by the dog in order to maintain cleanliness of the local park or facility, and to safeguard the environment.

    Should the dog's safety become concerning, Wolfe Pack Dog Training reserves the right to terminate future training of the dog. Refunds will be considered at the trainer's discretion in this circumstance.

    Emergencies

    In the event of an emergency that requires veterinary care for your dog, both you and your emergency contact will be contacted. If contact is unable to be made, your dog will be taken to your preferred clinic if feasible, or in the event of a critical need, the closest emergency veterinary hospital, and further attempts will be made to obtain contact with you and your emergency contact.

    The dog owner is responsible for reimbursing costs of emergency veterinary care.

    Time and Payment

    Each training session or service shall be scheduled in advance. In consideration for Wolfe Pack Dog Training, LLC's services rendered, the dog owner agrees to provide payment immediately following each training or daycare session. Payment for boarding and board‑and‑train services is due at pickup, less the pre-paid deposit, unless otherwise agreed in writing.

    Payment may be made via cash, personal check, Zelle, Venmo, or credit/debit card. There is no processing fee for a credit card payment.

    Board-and-train and boarding services incur a non-refundable pre-paid 30% deposit due one week before the scheduled service period.

    Late payments incur a 30% fee after one week. After one month, a late payment will be sent to a collections agency.

    A fee equal to 50% of the scheduled session or service cost will be charged for same-day cancellations, no-shows, or late arrivals exceeding 15 minutes without prior communication.

    Weather

    Training sessions and group classes may be rescheduled due to extreme heat, lightning, storms, or other inclement weather. Board-and-train and daycare dogs may have their outdoor training and play schedule modified accordingly during unsafe weather.

    Injury and Indemnification

    If the dog identified in this form accidentally dies, becomes ill, injured, or lost, dog owner waives and releases Wolfe Pack Dog Training, LLC, and any local park or public facility used during group class services, from any claim arising from such an incident.

    If the dog bites or injures the trainer or any other human or animal during a training session, the dog owner is responsible for any resulting injury.

    Additionally, the dog owner shall hold harmless and indemnify Wolfe Pack Dog Training, LLC, and the local park or other facility if applicable, against any and all costs, expenses, losses, liabilities, and claims arising out of or relating to any acts of the dog, except if arising out of the negligence or intentional misconduct on the part of the dog trainer.

    During group class services, the owner assumes the risk of being in a public environment. Wolfe Pack Dog Training, LLC is not responsible for the actions of third-party dogs, people, or other animals in public environments. The dog owner understands and accepts that working with animals involves inherent risks.

    Governing Law

    The terms of this Agreement shall be governed by and construed in accordance with the laws of the State of Florida and Brevard County, not including its conflicts of law provisions.

    Severability

    If any provision of this Agreement is held to be invalid or unenforceable in whole or in part, the remaining provisions shall not be affected and shall continue to be valid and enforceable as though the invalid or unenforceable parts had not been included in this Agreement.

    Entire Agreement and Amendment

    This Agreement contains the entire understanding between the dog owner and Wolfe Pack Dog Training, LLC, and supersedes and cancels all prior agreements by the Parties. This Agreement may be amended or modified only by a written agreement signed by all the Parties.
