  • In consideration of the acceptance of this application for booth space, the undersigned hereby elects to, and does, release Silver Creek Methodist Church and all respective officials and employees from any and all claims, demands, rights, or causes of action of any kind or nature-whether known or unknown, foreseen or unforeseen-that the undersigned has ever had, now has, or may hereafter have, arising from, related to, or in any way connected with any injuries, losses, damages, or the results thereof, which have been or may hereafter be sustained by the undersigned as a result of, in connection with, or arising out of participation in The Silver Creek Methodist Church Lord's Acre. 

    By signing below, I acknowledge that I have read and understand the above release and the "Lord's Acre Rules” and agree to abide by all terms and conditions. 

    If you have any pictures of your booth setup, please email them to Dena at dmhall20@gmail.com so we can share them on our church Facebook page.

    Please note that all booths are outdoors. You are required to provide your own tables, chairs, canopy, etc.
