Dental Referral Acknowledgment:
By signing below, I understand and agree to the following:
- It is my responsibility to contact my regular veterinary hospital and request that they email my pet’s SOAP notes, medical records, and any relevant diagnostics to Spay Now for review. Spay Now will not contact my veterinary hospital to obtain these records. Our email is spaynowsurgery@gmail.com
- It can take up to 7 business days once we have received both the records and the form to reach out.
- Once Spay Now has received and reviewed my pet’s records, the team will contact me via email regarding next steps.
- Dental appointment availability is extremely limited, and priority is given to the most urgent cases.
- Submission of this referral form does not guarantee that my pet will be scheduled for a dental procedure.
- If the doctor determines that Spay Now is able to perform my pet’s dental procedure, I will be contacted via email to schedule an appointment.
- A non-refundable deposit of $100 is required to book the appointment.
- Pets over the age of 8 years old are required to have Pre-operative bloodwork.
- Our dental prices range from $675-$1225
- We offer full mouth dental radiograph for $250 at your request.
