Affidavit of Supporting Tax Documentation

Sworn statement attesting to the accuracy and completeness of tax documents for Family First Tax Services I am providing this affidavit in support of my federal and/or state income tax return for the tax year 2025_____. I affirm that the information and documents I have provided to Family First Tax Services are true, correct, and complete to the best of my knowledge. I understand that certain documents used to prepare my tax return, including but not limited to income statements, receipts, or records, may not be available through traditional forms such as a Form W-2, Form 1099, or other official statements. I hereby certify that the information provided regarding my income, expenses, dependents, filing status, and any tax credits or deductions claimed is accurate and supported by records in my possession or by my personal knowledge. I understand that I am solely responsible for the accuracy of the information reported on my tax return and for maintaining adequate documentation in the event of an inquiry by the Internal Revenue Service or any state taxing authority. I acknowledge that Family First Tax Services has prepared my tax return based solely on the information I provided and is not responsible for any omissions, inaccuracies, penalties, interest, or additional taxes resulting from incorrect or incomplete information. I make this affidavit voluntarily for the purpose of supporting the preparation and filing of my tax return. Further Affiant sayeth not.
