The Adopter acknowledges that the above-named dog has demonstrated dog aggression, dog reactivity, and/or behaviors indicating the dog cannot safely cohabitate with other dogs at this time. The Rescue has discussed the dog’s
known history, behaviors, management needs, and recommendations at length with the Adopter.
The Adopter understands and agrees that:
• This dog is to remain an only dog in the household unless otherwise directed by a certified professional trainer or veterinary behaviorist.
• The dog should not freely interact with unfamiliar dogs.
• Proper supervision, containment, and management are required at all times.
• Behavioral outcomes cannot be guaranteed by the Rescue.
The Adopter accepts full responsibility for the dog immediately upon adoption, including all future handling, management, training, containment, and safety precautions involving the dog and any interactions with other animals or
people.
The Adopter further agrees to assume all liability for any incidents, injuries, damages, losses, or expenses occurring after adoption involving the dog. The Adopter agrees to release, indemnify, and hold harmless Notorious D.O.G. Dog
Rescue, including its board members, volunteers, fosters, officers, and representatives, from any and all claims, liabilities, damages, costs, or legal expenses arising after transfer of ownership and custody of the dog.
By signing below, the Adopter confirms that they fully understand the disclosed behavioral concerns, have had the opportunity to ask questions, and voluntarily choose to adopt the dog with full knowledge of these risks and responsibilities.
