WAIVER & RELEASE OF LIABILITY

I, the undersigned parent/legal guardian, give permission for my child to participate in the Bears Youth Football Camp held at ALA Ironwood High School Field. I also understand that participation in football and athletic activities involves inherent risks, including but not limited to falls, collisions, injuries, illness, and other risks associated with physical activity.

I voluntarily assume all risks related to my child's participation and release, waive, and hold harmless Bears Youth Football Club, ALA Ironwood High School, its owners, employees, staff, coaches, volunteers, sponsors, and affiliated organizations from any and all claims, liabilities, damages, losses, or expenses arising from participation in the camp, except as otherwise provided by law.

I certify that my child is physically able to participate in camp activities and that I have disclosed any medical conditions, allergies, or special needs that may affect their participation. In the event of an emergency, I authorize camp staff to secure appropriate medical treatment for my child if I cannot be reached immediately.
