2026 Summer Skatium Birthday Party Inquiry Form

General Party Information

1. The party package includes 45-min in the party room and 60-min on the ice for up to 50 guests, one round of cups, napkins, and cake size plates. If serving food, such as pizza, bring full-size plates with you. Skate rental is an additional fee of $3 per guest that needs skates.
2. Hosts should not arrive more than 5 min prior to the start of the allotted time. The 45-min room time includes set-up and break-down time so it is best to keep décor to a minimum; if host will need time to set up, they should indicate on their invites a party start time 10-15 mins after the actual room time begins.
3. A party assistant is assigned to your party to help with loading and unloading your party supplies, room set-up, serving of food/cake, clean-up, and skate rental. The host may request the party assistant refrain from helping with these tasks and solely do time check-ins. If your party is a broomball party, the assistant will provide broomball supplies and place the nets on the ice.
4. A non-refundable fee of $295 is due at the time of booking. We do not hold dates, once booked the date is firm. We do not move or cancel parties. The skate rental is due on the day of your party. Your party assistant will take your final payment.
5. We allow up to five skate aids on the ice, which your party assistant will provide upon request. Guests must share skate aids.
6. We do not allow piñatas, confetti or similar material. No helium ballons or other floating décor, if your balloons or décor set off the beam fire alarm, this will immediately end your party.
7. No alcohol, smoking, or vaping allowed in the facility.
8. No food or drinks allowed on the ice surface.
9. No one should enter the ice surface without skates (unless the party is a broomball party).
10. We do not split the ice between activities (i.e., hockey game on one side and regular skating onthe other) as this can be dangerous.
11. Your party will have access to the Skatium’s sound system and can use the radio or bring theirown playlist. Please bring your own aux adapter.
12. When you book, your receipt will contain attachments of room and rink rules, reminders, andguidelines, please review these items carefully to help your day run smoothly.

SKATIUM ICE ARENA WAIVER

The Skokie Park District and Skatium Ice Arena (“Park District”) are committed to providing safe skating facilities and programs and holds the safety of participants in high regard. The Park District continually strives to reduce risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors must recognize that there is an inherent risk of injury. Participants, or the parents/guardians of participants, are solely responsible for determining if you/your minor child/ward are physically fit and/or adequately skilled for the activities associated with this reservation. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical/skating activity.

PLEASE UNDERSTAND AND RECOGNIZE THAT RINK STAFF ARE NOT RESPONSIBLE FOR PROVIDING SUPERVISION OR ASSESSING YOUR SKATING SKILLS, OR THAT OF YOUR MINOR CHILD; RATHER, RINK STAFF ARE RESPONSIBLE FOR ENFORCING SAFETY RULES AND RESPONDING TO EMERGENCIES. PARTICIPANTS, OR PARENTS/GUARDIANS ARE SOLELY RESPONSIBLE FOR SUPERVISION OF ANY AND ALL ACTIVITIES CONTEMPLATED BY THIS AGREEMENT. CHILDREN 10 YEARS OF AGE AND YOUNGER MUST BE SUPERVISED AT ALL TIMES BY A RESPONSIBLE PERSON, AT LEAST 16 YEARS OF AGE OR OLDER. NEVER LEAVE ANY CHILD WITH POOR SKATING SKILLS OR 10 YEARS OF AGE AND YOUNGER UNATTENDED. WARNING OF RISK

Skating and other ice activities challenge and engage the physical, mental and emotional resources of each participant. However, despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury, including but not limited to head/brain injury and spinal cord injury. Understandably, not all hazards and dangers can be foreseen. The very nature of skating and ice activities are hazardous and risky, including but not limited to fatigue and overexertion, poor skating skills, failing to avoid dangerous areas, failing to follow rules and regulations, failure of staff to locate victims and/or delay in emergency response time, horseplay, including horseplay that can cause skaters to fall striking the rink boards or fall awkwardly onto the ice, inadequate supervision or instruction, lack of conditioning, becoming disoriented, striking other skaters, striking ones head, slips or falls, and all other circumstances inherent to ice activities. In this regard, it must be recognized that it is impossible for the Skokie Park District to guarantee absolute safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK
Please read this form carefully and be aware that in consideration for this facility reservation, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages, or loss which you, your guests, your program participants, and your minor children/wards might sustain as a result of participating in any and all activities connected with and associated with the use of Skokie Park District and Skatium Ice Arena skating facilities and programs. I recognize and acknowledge that there are certain inherent risks of physical injury to patrons of skating facilities, and I voluntarily agree to assume the full risk of any injuries, damages, or loss, regardless of severity that I, my minor child/ward, program participants, or my guests or their minor children/wards may sustain as a result of participating in any and all activities and programs connected with or associated with this facility reservation. I further recognize and agree that I am solely responsible for supervising any minor children/wards present at my reservation. I additionally agree to ensure that all children present for my reservation under the age of 10 are supervised by a responsible adult, at least 16 years of age. I further agree to waive and relinquish all claims, I, my participants, my guests, or my minor child/ward may have (or accrue to me, my guests, or my child/ward) as a result of use the Park District’s skating facilities and programs, against the Park District, including its officials, agents, volunteers and employees. I do hereby fully release and forever discharge the Park District from any and all claims for injuries, damages or loss that I, my guests, program participants, or my minor child/ward may have, or which may accrue to me, my guests, participants, or my minor child/ward, and arising out of, connected with, or in any way associated with use of the Park District’s skating facilities and programs.
