ASSUMPTION OF RISK

I understand that participation in the Young Adult Field Day may involve physical activities, including but not limited to games, recreational sports, relay races, outdoor activities, and other group activities that may involve inherent risks of injury.

I voluntarily choose to participate in this event and acknowledge that participation may involve risks including, but not limited to:

Slips, trips, and falls
Physical contact with other participants
Muscle strains, sprains, and other injuries
Weather-related conditions
Injuries resulting from recreational activities
Other unforeseen risks associated with participation
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of other participants.

RELEASE OF LIABILITY

In consideration of being permitted to participate in the Young Adult Field Day, I hereby release, waive, discharge, and covenant not to sue:

Love and Faith Christian Fellowship
Its pastors, ministers, employees, volunteers, officers, directors, agents, and representatives
Event organizers, sponsors, and activity leaders
from any and all liability, claims, demands, actions, causes of action, damages, losses, costs, or expenses arising out of or related to any injury, illness, property damage, or loss that may occur as a result of my participation in the event, except where prohibited by applicable law.

MEDICAL AUTHORIZATION

I certify that I am physically able to participate in the activities offered during the event.

In the event of an emergency, I authorize event staff and volunteers to obtain emergency medical treatment for me if I am unable to provide consent myself.

I understand that I am responsible for any medical expenses incurred as a result of participation in this event.

PHOTO, VIDEO, AND MEDIA RELEASE

I grant permission to Love and Faith Christian Fellowship and its representatives to photograph, record, and/or videotape me during the event.

By signing this wavier, I authorize the use of my image, likeness, voice, and statements in promotional materials, social media, websites, livestreams, publications, and other ministry-related communications without compensation.

CODE OF CONDUCT

I agree to conduct myself respectfully and responsibly throughout the event. I understand that organizers reserve the right to remove any participant whose behavior is disruptive, unsafe, inappropriate, or inconsistent with the values and expectations of the event.

ACKNOWLEDGMENT

I have carefully read this Waiver, Release of Liability, and Media Release Form. I fully understand its contents and sign it voluntarily of my own free will.

Appropriate Attire for Field Day
Participants are encouraged to wear comfortable, modest, and activity-appropriate clothing suitable for outdoor games and physical activities. Athletic wear, t-shirts, shorts, joggers, and sneakers are recommended. Closed-toe athletic shoes are required for participation in most activities. Please avoid clothing with offensive language or images, excessively revealing attire, and footwear that may pose a safety risk, such as sandals, slides, or high heels. Dress comfortably, stay hydrated, and be prepared for the weather.
