    Kia ora and welcome to the official 2026 registration form for Models in Practice — Auckland's small group register of supervised aesthetic treatment members.

    You're about to join a curated group of people who sit with us across the year while our injectors train in new aesthetic treatments, all under Nurse Practitioner supervision.

    What's included in your membership:

    • Access to our 2026 workshop calendar to book the treatments and dates that suit you
    • 3 to 4 supervised treatment sessions across the year
    • Anti-wrinkle, dermal filler, skin boosters, PRP ·

    Full medical oversight from our Nurse Practitioner · Pre-treatment consultation, treatment-day assessment, and full aftercare

    Your investment: $500 per year, or $10 per week.

    Here's what happens next:

    This form (around 5 minutes): a few short health questions and your payment details.

    Next: we'll send you a suitability screening to complete in your own time, reviewed by our supervising Nurse Practitioner within 5 working days.

    Then: your workshop calendar access is activated and you can start booking.
    If our Nurse Practitioner determines you're not clinically suitable, your full membership fee is refunded. No catch, no questions.

  • Your Models in Practice membership is $500 NZD per year. You can pay your annual fee upfront by entering your details below or set up a weekly payment plan from $10 per week through our membership provider.

    Your membership year runs for 12 months from today's date. Members are typically matched to a minimum of three and a maximum of four training sessions per annual membership, subject to clinical suitability and training calendar availability.
