Kia ora and welcome. This short form registers you as a Models in Practice member. It takes around 5 minutes to complete and includes a few brief health questions before we collect your payment. Once you've completed registration, we'll send you a longer medical screening and consent form, which is reviewed by our supervising Nurse Practitioner before your matching eligibility is activated. If our Nurse Practitioner determines you are not clinically suitable for the programme, your full membership fee is refunded.

Before we move to payment, please answer four quick questions about your eligibility. These help us make sure the programme is right for you. If your answers indicate that the programme is not suitable, we'll let you know before any payment is processed.

Tick the treatment categories you are interested in being matched to during your membership. You can update your preferences at any time by contacting us. Final matching is always subject to clinical suitability assessed by the supervising Nurse Practitioner. Full information about each treatment, including risks, benefits and contraindications, will be provided in your medical screening form.

Your Models in Practice membership is $500 NZD per year. You can pay your annual fee upfront by entering your details below or set up a weekly payment plan from $10 per week through our membership provider.

Your membership year runs for 12 months from today's date. Members are typically matched to a minimum of three and a maximum of four training sessions per annual membership, subject to clinical suitability and training calendar availability.
