1. Acknowlegement of Activities and Risks

I, the undersigned parent or legal guardian of the above-named child, grant permission of the above-named child, grant permission for my child to participate in all activities offered by YMM - Catholic Youth Ministry's Vacation Bible School, including but not limited to sports, arts and crafts, and outdoor games.
I understand that these activities involve inherent risks, including but not limited to physical injury, or property damage, and I acknowledge that YMM-Catholic Youth Ministry's Vacation Bible School cannot eliminate all risks.

2. Health and Medical Authorization

I confirm that my child is in good health and able to participate in the activities. In case of a medical emergency, I authorize YMM-Catholic Youth Ministry's Vacation Bible School staff to seek and provide necessary medical treatment for my child, including but not limited to first aid, hospital visits, or administration of prescribed medications. I agree to cover any medical expenses occurred.

3. Liability Waiver

In consideration of my child's participation, I hereby release, waive, and discharge YMM - Catholic Youth Ministry, St. Paul Parish, and the Diocese of St. Paul, its staff, volunteers and affiliates from any and all liability claims, or damages arising from my child's participation in the activities, except in cases of gross negligence or willful misconduct by Vacation Bible School. I agree to indemnify and hold harmless YMM - Catholic Youth Ministry, Vacation Bible School, St. Paul Parish, and the Diocese of St. Paul from any claims brought by or on behalf of my child.

4. Photo/Video Release

I grant YMM - Catholic Youth Ministry's Vacation Bible School permission to use photographs or videos of my child taken during YMM - Catholic Youth ministry's Vacation Bible School activities for promotional purposes, including posting on the website, social media, or printed materials, unless otherwise indicated.

5. Behavior and Pick-Up Authorization

I understand that my child is expected to follow YMM-Catholic Youth Ministry's Vacation Bible School rules and behave appropriately. YMM-Catholic Youth Ministry's Vacation Bible School reserves the right to dismiss my child from the program without refund for serious or repeated behavior issues.

Acknowledgement and Signature: I have read and understand this release and waiver form. I voluntarily sign this document, agreeing to its terms and terms and conditions, to allow my child to participate in YMM - Catholic Youth Ministry's Vacation Bible School's program.
