Snap Fitness Sioux Center – Waiver & Membership Agreement

I understand that my 90-day Summer Membership begins on the date I submit this registration and that the 90-day membership period will not be extended due to delays in obtaining an access card, signing required documents, or activating gym access. I understand that I must complete and sign all required membership documents, including the Membership Agreement and Liability Waiver, before access to the facility will be granted. I understand that a valid email address must be provided so these documents can be sent electronically for signature, or I may complete the documents in person at Snap Fitness with a staff member. I understand that all Summer Membership purchases are final and non-refundable. No refunds, credits, extensions, or transfers will be provided for unused time, failure to complete required documents, failure to obtain an access card, or inability to use the facility during the membership period. By submitting this form, I acknowledge and agree to these terms.
