Choose To Stay recognizes that those who have lost a family member to suicide may face many
challenges, which may include financial. We offer to assist with funeral or funeral related expenses,
counseling, or other expenses deemed needed by the Choose To Stay Board of Directors.

In order to be considered for financial assistance, you:

• Must be the individual requesting assistance.
• Must be the individual completing the application.
• Must be 18 years of age or older.
• Must reside in Mecosta County, Michigan.
• Must be an immediate family member of the deceased.
• Must be struggling with suicide ideation.

You must also provide the following documentation:

• A copy of a current Michigan Driver's license or State Id.
• Verification of current address.
• Name and number of the business where you are receiving services. If assistance for counseling
  please advise the counselor we will be contacting them to verify services are being received.
• A copy of your family members obituary.

This application and accompanying documentation must be completed and returned to the following
address within one (1) year of your family members passing. No documentation will be returned to you.
