Alumni/Volunteer/Guest/Vendor Contact, Participation, and Release Agreement
By submitting my information or participating in alumni activities, I agree as follows:
1. Voluntary Participation. I voluntarily provide my contact information to Healing Springs, LLC (“HSR”), and understand that I am voluntarily self-identifying as a former client/alumnus/volunteer/guest/vendor of HSR by participating in any alumni event.
2. Confidentiality. I agree to respect the privacy of other clients/alumni and to not disclose, share, reproduce, or misuse personal information obtained through alumni communications or events. This includes, without limitation, refraining from screenshots, recordings, or redistribution of communications, images, or identifying information without express permission. I will not use alumni communications for unauthorized solicitation or promotion. I understand HSR may share my contact information with HSR personnel and verified alumni for alumni-related purposes. I acknowledge that confidentiality cannot be guaranteed in group settings or communications once information is voluntarily shared.
3. Code of Conduct. I agree to act respectfully at alumni events and in alumni communications. I understand and agree that HSR may moderate communications, remove individuals from groups or events, restrict participation or take other appropriate action at its sole discretion for any conduct that it deems unsafe, disruptive, or inconsistent with this Agreement. I understand and agree that the following are strictly prohibited at all alumni events and in alumni communications (including virtual platforms):
Possession, use, or carrying of firearms, weapons, or any item that could be used to cause harm, whether or not legally permitted;
Audio, video, or photographic recording of any kind without the prior express permission of HSR and all individuals being recorded;
Harassment, threats, intimidation, or disruptive behavior;
Unauthorized solicitation, promotion, or commercial activity.
Inappropriate attire (tight clothes, revealing, low cut, bathing suits, halters, short skirts, etc.)
HSR reserves the right to remove any individual from an event or communication platform and to take further action, including notifying appropriate authorities, if this policy is violated.
4. Consent to Contact. I consent to be contacted by phone, email, text message, or mail about alumni-related matters. By selecting “Yes” below and providing my mobile number, I consent to receive alumni-related text messages from HSR. I understand that message and data rates may apply, and am aware that SMS consent is optional and not required to participate in alumni activities. I understand I may opt out of receiving text messages at any time by contacting HSR with my request.
5. Consent to Photograph. By selecting “Yes” below, I hereby authorize HSR to use my name, image, and/or likeness, from alumni activities for HSR and alumni-related purposes, including alumni communications, social media, website content, newsletters, and promotional materials. I understand media consent is optional and not required to participate in alumni activities. I may revoke future consent by emailing HSR, and I understand that revocation may not affect materials already published or used before HSR receives my request.
6. Substance-Free Requirement. I understand that all alumni events are strictly drug- and alcohol-free. I will not possess, use, distribute, or be under the influence of drugs or alcohol at any alumni function. I will not use any tombaco or nicotine products or devices (vapes) on campus or do anything that could jeopardize our clients in treatment.
7. No Treatment Relationship. I understand that participation in alumni programs does not establish, create, continue, or renew any clinical, therapeutic, medical, or therapist-patient relationship with HSR. I understand that alumni events and communications are for social and support purposes only.
8. Medical and Emergency Disclaimer. If I experience a medical or psychiatric emergency, relapse, or other urgent situation, I understand that I should call 911, go to the nearest emergency room, or contact a qualified healthcare provider. I understand that HSR is not responsible for monitoring alumni communications for emergencies and may not see or respond to messages immediately. In the event that emergency services must be called on my behalf, I hereby consent to emergency medical treatment if I am injured while attending the alumni activity and accept full responsibility for all medical expenses incurred as a result of my attendance and participation.
9. Release of Liability. To the fullest extent permitted by law, I release and hold harmless HSR, its owners, officers, employees, contractors, and affiliates from claims, liabilities, damages, or injuries arising out of or related to my participation in alumni activities, including those resulting from the actions of other participants.
10. Modification and Termination. I understand that HSR may modify alumni policies or discontinue alumni programs at any time. I understand that participation may be revoked, in HSR’s sole discretion, for violations of this Agreement or other safety, confidentiality, or conduct concerns.
11. Governing Law. This Agreement shall be governed by the laws of the State of Texas.
By signing below, I acknowledge that I have read, understand, and agree to this Alumni Contract, Participation, and Release Agreement. I confirm that I am a former client/alumnus/guest of HSR, that my participation in alumni events are voluntary, and that the information I provided is accurate.
