I hereby give my approval for my child’s participation in any and all activities prepared by DC Hoop School, during the selected camp. In exchange for the acceptance of said child’s candidacy by DC Hoop School. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Point Forward Basketball Group. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against DC Hoop School. including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to DC Hoop School and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

In the event of an emergency, I hereby authorize the staff of DC Hoop School Basketball Clinic to obtain medical treatment for my child. I understand that every effort will be made to contact me before initiating treatment.

I, the undersigned, hereby acknowledge and agree that participation in the DC Hoop School Basketball Clinic involves inherent risks, including, but not limited to, the risk of physical injury. In consideration of being allowed to participate in the Clinic, I agree to the following:

1. Assumption of Risk: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Clinic or others, and assume full responsibility for my participation.

2. Release and Waiver: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless DC Hoop School, its officers, officials, agents, and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releases”), from any and all claims, demands, losses, and liability arising out of or related to any injury, disability, or death I may suffer, or loss or damage to person or property, whether arising from the negligence of the Releases or otherwise, to the fullest extent permitted by law.

3. Medical Treatment: I give my consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during the Clinic.

4. Compliance: I agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

I, the undersigned, hereby grant DC Hoop School and its representatives the right to take photographs and videos of me/my child in connection with the DC Hoop School Basketball Clinic. I authorize DC Hoop School to use and publish the same in print and/or electronically.

I agree that DC Hoop School may use such photographs or videos of me/my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
