This form is to be completed by participants in the Pop Warner program that are enrolled in home school. This form must be accompanied by a progressing progress report or report card from an accredited governing body to be eligible for play after the October 15 deadline. That report must be dated between September 1 and October 15. If the progress report or report card isn't received within the dates noted above the player shall be found ineligible for the rest of the season.

As the above named participant is enrolled in home school, the undersigned agrees to the terms shown above as a condition to being passed on the National roster.

1. This form must be completed in full and signed by the parent/guardian.
2. One acceptable progress report must be turned in during the time frame specified above-between September 1 and October 15.
3. If no progress report is received, the player is ineligible for the remainder of the season.
4. This form and the one progressing progress report must be in book at all times.
5. If progressing progress is received in the designated timeframe the player is eligible for the remainder of the season.
6. If a player is found ineligible and plays, the team and coach shall be subject to penalties as outlined in the National Pop Warner Rule Book.
7. At the end of the designated time period, a League Board member or member of the League Scholastic team must certify eligibility for the rest of the season.
8. The Head Coach and/or Parent/Guardian will be required to come before the PW board if found allowing an ineligible child to participate in any game.
