I, the undersigned parent or legal guardian of the above-named mini, hereby grant permission to Soul Dive LLC, its members, representatives, videographers, photographers, producers, agents, successors, and assigns to photograph, film, record, and otherwise capture the minor's image, likeness, voice, performance, appearance, and participation in connection with the production of a music video and promotional materials.

I understand and agree that the minor's image, voice, performance, and likeness may be used, reproduced, edited, altered, copied, exhibited, published, distributed, displayed, transmitted, broadcast, streamed, and otherwise exploited in any manner and in any media now known or later developed, including but not limited to:

Music Videos, YouTube, Social Media Platforms, Streaming Services, Websites, Digital Advertising, Promotional Materials, Press Releases, Concert and Event Promotions, Merchandise Promotion, Future Marketing and Publicity Materials

I understand that these materials may be viewed publicly thorughout the world and may remain indefinitely.

No Compensation

I acknowledge that participation is voluntary and that neither I nor the minor is entitled to compensation, royalties, ownership rights, or future payments arising from the use of the minor's image, voice, likeness, or performance unless otherwise agreed to in writing.

Release of Claims

I hereby release and discharge Soul Dive LLC, its members, representatives, videographers, produces, employees, contractors, agents, successors, and assigns from any claims, demands, damages, liabilities, or causes of action arising out of or related to the use of the minor's image, voice, likeness, performance, or participation as authorized by this Release.

Representaions

I represent and warrant that:

1. I am the parent or legal guardian of the minor identified above.

2. I have full legal authority to execute this Release.

3. The minor is participating voluntarily.

4. I have read and understand this Release and agree to its terms.
