    Completing this form will register your child/ren for the 2026/2027 school year. Families who register prior to September 1, 2026 are ensured a space provided there are no unforeseen circumstances and can begin the program once the $50 registration fee has been paid. Once registered, families can begin utilizing the program at any time during the school year. Please ensure you have noted the correct school phone number to use for drop off and pick up of your child(ren).

    Golden Rule Programs (hereinafter called the “program”) of the first part, located at Roland Michener School (4215 Degeer Street, Saskatoon, SK)

    And

    (Name(s) of Parents or Guardians) listed above.

    The Program and the Parent(s) agree as follows:

    1. The program agrees to provide supervision during the hours of 7:30 am to 8:53 am and 3:00 pm to 6:00 pm, Monday to Friday, on all regular school days. 

    2. If the child attends the program on a full time regular basis the parent/guardian agrees to notify the program of the days and times, via text, that the child will not be in attendance. If the child does not attend the program on a regular basis the parent agrees to notify the program, via text, in advance, re: the child's attendance at the program.  More importantly it is crucial for the child/children to know when they are attending as we do not pick children up from their classrooms.  Always a good idea to let the classroom teacher know if you think your child/children will need guidance at the end of the school day.

    3. The parent agrees to pay the program a $50.00 registration fee per family, per school year, which must be paid within 72 hours of completing the registration form or the form/application will be deleted. If you have filled out the form but not paid this fee, your child(ren) cannot attend. This can be paid immediately following the completion of this form and can be paid via EFT. Instructions down below.

    In addition, the parent agrees to pay a fee of $5.25/hour to the nearest minute that each child is in care before school and $5.25 for each hour to the nearest minute that each child is in care after school. There is a minimum one hour charge for each before and afternoon care, no matter the amount of time your child is in attendance. After 60 minutes of care, the fee is .0875 cents per minute.  The program offers a family rate of $10.50 per hour for families with two or more children in attendance.  

    4. The total amount owing for the month will be available within three school days after the last day of each month. This invoice will be emailed to you at the email address provided upon registration. This is sent to the email(s) provided and payment is expected so please ensure it is correct.   It is sent as an attachment and includes a breakdown of the minimum one hour of care on each end of the day and any additional minutes used as well as any late fees.   Payment must be made on or before the payment due date set out in your invoice. Parents will accept a late payment charge of $5/day for every day payment is not received beyond the due date. This late fee will be added to the following month's fees. Payments not received by the last day of the following month will result in immediate suspension from the program until fees are paid in full.

    5. The parent agrees to provide the names of all persons other than the parent to whom the program may release the child. The parent agrees that the program shall have no responsibility before releasing the child to that person, other than requesting that person to produce identification. The parent agrees that as soon as the child is released to that person, the program is not responsible for the child in any manner whatsoever.  These changes can be texted to our school phone from one of the registered parents/guardians.

    6. Child Information, Individual Needs and Program Limitations
    The Parent/Guardian agrees to provide the Program with complete and accurate information regarding the child’s individual needs, including allergies, medical conditions, behavioural needs, and any other information that may require special care, attention, supervision, or accommodation.
    The Program cannot be responsible for an injury, illness, or other incident resulting from the Parent/Guardian’s failure to provide complete and accurate information regarding the child.
    The Program reserves the right to decline admission or continued attendance when, based on the information available to the Program, a child’s cognitive, emotional, physical, medical, or behavioural needs cannot reasonably be supported within the Program’s regular staffing model and available resources.
    Golden Rule Programs does not provide additional one-to-one staffing or specialized supports and does not qualify for or provide Educational Assistant (EA) grants or one-to-one staffing supports.
    The Program is generally unable to accommodate children who require individual classroom support or one-to-one assistance in order to participate safely and successfully.
    Our goal is always to meet each child’s basic needs and provide a safe, positive environment. However, we recognize that our programs may not be suitable for every child and that there are limitations to the level of individualized support we can provide.
    The safety, health, and well-being of all children entrusted to our care must be maintained. If a child requires one-to-one staffing or supports beyond what can reasonably be provided within our regular staffing model in order to participate safely, or if the child’s needs significantly interfere with the Program’s ability to safely supervise the other children, we may be unable to provide care.
    Parents/Guardians who have questions regarding their child’s individual circumstances are encouraged to contact the Program before registering or paying the $50.00 registration fee so that we can discuss whether our Program is able to meet the child’s needs.

    7. Late Pick-Up
    The Program closes at 6:00 p.m.
    A $30.00 late fee per half hour, or portion thereof, will be charged for any child picked up after 6:00 p.m.
    Program staff will attempt to contact the Parent/Guardian and the other emergency/authorized contacts listed on the registration form to arrange immediate pick-up.
    If no contact has been made with a Parent/Guardian or listed contact by 6:30 p.m., the Program will follow its established procedures for ensuring the child’s safety and arranging appropriate assistance.
    If you know you are going to be late, you must text the school phone for your location as soon as possible.
    Habitual late pick-up may result in the child no longer being permitted to attend the Program.

    8. Independent Dismissal
    The Parent/Guardian agrees to advise the Program in writing or by text if the child is permitted to leave the Program’s premises without a Parent/Guardian or other authorized adult, including the specific time the child is to be dismissed.
    Once the child has left the Program’s premises at the time and in the manner authorized by the Parent/Guardian, the Program is no longer responsible for the child.

    9. Risk and Liability
    The Parent/Guardian acknowledges that participation in a before- and after-school program involves ordinary risks associated with children’s activities, including activities on playgrounds and other areas of the school premises.
    The Parent/Guardian acknowledges these inherent risks and agrees that the Program is not responsible for injuries, losses, or incidents except to the extent that responsibility cannot lawfully be excluded or limited.

    10. School Cell Phone and Communication
    The school cell phone number for daily communication at your child’s school will be given in your confirmation of registration.
    This number is intended for:
    Drop-off and pick-up communication;
    Attendance and schedule changes;
    Day-to-day communication with Program staff; and
    Other matters directly related to your child’s attendance that require communication with the school location.
    The school cell phone is not intended for billing questions, payment issues, or changes to billing/email information. Please contact the Administration Office regarding these matters.

    11. School Closure Days
    After School Program does not operate on non-school days, including:
    School breaks;
    Professional development days;
    Statutory holidays; and
    Other days when the school is not in session.

    **The $50 per family registration fee must be paid before registration is complete and your child/children can attend.  Your child/ren can begin attending the program upon completion of this form and payment of registration fee.     1. The program agrees to provide supervision during the hours of 7:00 am to 8:40 am and 3:10 pm to 6:00 pm, Monday to Friday, on all regular school days. 

    2. If the child attends the program on a full time regular basis the parent/guardian agrees to notify the program of the days and times, via text, that the child will not be in attendance. If the child does not attend the program on a regular basis the parent agrees to notify the program, via text, in advance, re: the child's attendance at the program.  More importantly it is crucial for the child/children to know when they are attending as we do not pick children up from their classrooms.  Always a good idea to let the classroom teacher know if you think your child/children will need guidance at the end of the school day.

If a child requires one-to-one staffing or supports beyond what can reasonably be provided within our regular staffing model in order to participate safely, or if the child’s needs significantly interfere with the Program’s ability to safely supervise the other children, we may be unable to provide care.
    Parents/Guardians who have questions regarding their child’s individual circumstances are encouraged to contact the Program before registering or paying the $50.00 registration fee so that we can discuss whether our Program is able to meet the child’s needs.

    7. Late Pick-Up
    The Program closes at 6:00 p.m.
    A $30.00 late fee per half hour, or portion thereof, will be charged for any child picked up after 6:00 p.m.
    Program staff will attempt to contact the Parent/Guardian and the other emergency/authorized contacts listed on the registration form to arrange immediate pick-up.
    If no contact has been made with a Parent/Guardian or listed contact by 6:30 p.m., the Program will follow its established procedures for ensuring the child’s safety and arranging appropriate assistance.
    If you know you are going to be late, you must text the school phone for your location as soon as possible.
    Habitual late pick-up may result in the child no longer being permitted to attend the Program.

    8. Independent Dismissal
    The Parent/Guardian agrees to advise the Program in writing or by text if the child is permitted to leave the Program’s premises without a Parent/Guardian or other authorized adult, including the specific time the child is to be dismissed.
    Once the child has left the Program’s premises at the time and in the manner authorized by the Parent/Guardian, the Program is no longer responsible for the child.

    9. Risk and Liability
    The Parent/Guardian acknowledges that participation in a before- and after-school program involves ordinary risks associated with children’s activities, including activities on playgrounds and other areas of the school premises.
    The Parent/Guardian acknowledges these inherent risks and agrees that the Program is not responsible for injuries, losses, or incidents except to the extent that responsibility cannot lawfully be excluded or limited.

    10. School Cell Phone and Communication
    The school cell phone number for daily communication at your child’s school will be given in your confirmation of registration.
    This number is intended for:
    Drop-off and pick-up communication;
    Attendance and schedule changes;
    Day-to-day communication with Program staff; and
    Other matters directly related to your child’s attendance that require communication with the school location.
    The school cell phone is not intended for billing questions, payment issues, or changes to billing/email information. Please contact the Administration Office regarding these matters.

    11. School Closure Days
    After School Program does not operate on non-school days, including:
    School breaks;
    Professional development days;
    Statutory holidays; and
    Other days when the school is not in session.

    **The $50 per family registration fee must be paid before registration is complete and your child/children can attend.  Your child/ren can begin attending the program upon completion of this form and payment of registration fee. Simply text the school phone to advise our staff of their attendance.

    Golden Rule Programs (hereinafter called the “program”) of the first part, located at Bishop Filevich School (125-105th Street West Saskatoon, SK)

    And

    (Name(s) of Parents or Guardians) listed above.

    The Program and the Parent(s) agree as follows:

    1. The program agrees to provide supervision during the hours of 3:25 pm to 6:00 pm, Monday to Friday, on all regular school days.

    2. If the child attends the program on a full time regular basis the parent/guardian agrees to notify the program of the days and times, via text, that the child will not be in attendance. If the child does not attend the program on a regular basis the parent agrees to notify the program, via text, in advance, re: the child's attendance at the program.  More importantly it is crucial for the child/children to know when they are attending as we do not pick children up from their classrooms.  Always a good idea to let the classroom teacher know if you think your child/children will need guidance at the end of the school day.

    3. The parent agrees to pay the program a $50.00 registration fee per family, per school year, which must be paid within 72 hours of completing the registration form or the form/application will be deleted. If you have filled out the form but not paid this fee, your child(ren) cannot attend. This can be paid immediately following the completion of this form and can be paid via EFT. Instructions down below.
    In addition, the parent agrees to pay a fee of $5.25/hour to the nearest minute that each child is in care before school and $5.25 for each hour to the nearest minute that each child is in care after school. There is a minimum one hour charge for each before and afternoon care, no matter the amount of time your child is in attendance. After 60 minutes of care, the fee is .0875 cents per minute.  The program offers a family rate of $10.50 per hour for families with two or more children in attendance.  

    4. The total amount owing for the month will be available within three school days after the last day of each month. This invoice will be emailed to you at the email address provided upon registration. This is sent to the email(s) provided and payment is expected so please ensure it is correct.   It is sent as an attachment and includes a breakdown of the minimum one hour of care on each end of the day and any additional minutes used as well as any late fees.   Payment must be made on or before the payment due date set out in your invoice. Parents will accept a late payment charge of $5/day for every day payment is not received beyond the due date. This late fee will be added to the following month's fees. Payments not received by the last day of the following month will result in immediate suspension from the program until fees are paid in full.

    5. The parent agrees to provide the names of all persons other than the parent to whom the program may release the child. The parent agrees that the program shall have no responsibility before releasing the child to that person, other than requesting that person to produce identification. The parent agrees that as soon as the child is released to that person, the program is not responsible for the child in any manner whatsoever.  These changes can be texted to our school phone from one of the registered parents/guardians.

    6. The parent agrees to provide the program with all information with respect to the child's individual needs, including allergies and any special medical condition that requires special care or attention.
    The parent agrees that the program is not responsible for any injury or sickness due to the parent's failure to provide the required information.
    The parent agrees that the program has the right to decline admission to any child that the program is unable to support due to cognitive, emotional, physical or behavioral needs that are unable to be met by our typical staffing plan/ratio.
    Golden Rule Programs is unable to provide additional staffing or support for children with
    diverse needs, nor do we qualify for EA grants or 1:1 supports.
    Golden Rule Programs is unable to admit students who received individual classroom support. 

    It is our goal to do our best to meet children's basic needs, however, we realize that our programs may not be beneficial for every child and that there are limitations in what we can provide. It would be wrong to jeopardize the safety, health and well being of other children entrusted in our care or to offer a service that we cannot
    adequately provide. If a child cannot be safe in our program without one-on-one staffing, or their care interferes with our ability to care for other children, we regretfully will not be able to provide care for that child.  If you have questions regarding specifics on your child, please reach out prior to registering or paying the $50 fee and we will do our best to help you.  

    7. Late Pickup: A $30 late fee per half hour will be assessed for any pickups after 6:00pm. Program staff will attempt to text/call parents and all of the contact names on the registration form to arrange pickup. If no contact has been made with any listed contacts by 6:30 pm, arrangements will be made with Mobile Crisis to pick up your children.
    If you’re going to be late, please text our school cell phone designated to your school location. Habitual lateness will result in your children no longer being able to attend our program.

    8. The parent agrees to advise the program in writing or via text if the child is to be dismissed without supervision/parental accompaniment and at what time the child(ren) is to be dismissed. The program is not responsible for the child(ren) in any way whatsoever from the time the child leaves the premises of the program.

    9. The parent agrees that the program is released from all liability (including the physical and legal risk) incidental to the child (ren)'s supervision while the child (ren) are on the playground or the programs premises (In other words if the child is injured while in the care of the program the parent(s) will not take any legal action).

     10. The cell phone number for daily communication to the program at your child's school (BF) PLEASE CONTACT FOR INFO. This number can be used for drop offs and pickups, schedules or changes, and day to day communication with regular staff.

    The Program reserves the right to decline admission or continued attendance when, based on the information available to the Program, a child’s cognitive, emotional, physical, medical, or behavioural needs cannot reasonably be supported within the Program’s regular staffing model and available resources.
    Golden Rule Programs does not provide additional one-to-one staffing or specialized supports and does not qualify for or provide Educational Assistant (EA) grants or one-to-one staffing supports.
    The Program is generally unable to accommodate children who require individual classroom support or one-to-one assistance in order to participate safely and successfully.
    Our goal is always to meet each child’s basic needs and provide a safe, positive environment. However, we recognize that our programs may not be suitable for every child and that there are limitations to the level of individualized support we can provide.
