    We encourage the participation of volunteers who support our mission and are willing to contribute. The information provided through this form will be kept confidential and will help us determine the most satisfying and appropriate volunteer opportunity for you.

    The Christie Downs Community House endorses the following code of conduct to provide guidance to our employees and volunteers responsible for activities involving the community including children, young people under the age of 18 years and vulnerable people. The code serves to protect Centre users, children and vulnerable people and reduce any opportunities for abuse or harm to occur. Management, employees and volunteers all agree to abide by code of conduct. Employees must: - Be responsible for the administration and programs of Christie Downs Community House. - Maintain a duty of care towards others and be accountable for matters relating to the aim and purpose of Christie Downs Community House. - Establish and maintain a child safe environment in the course of their work (e.g. when conducting activities related to the aim and purpose of Christie Downs Community House. Employees and volunteers must: General Conduct: - Be fair, considerate and honest with others. - Operate within the rules and policies of the Christie Downs Community House and its activities. - Be professional in their actions through their use of language, presentation, manner and punctuality. - Resolve conflicts fairly and promptly and report and act on any breaches of these standards of behaviour through established procedures. - Maintain strict impartiality. - Act lawfully, honestly and exercise a reasonable degree of care and diligence in your dealings with the Centre. - Not smoke within the parameters of the Centre and not within 10 meters of the playground and dispose of buttes responsibly. - Conduct yourself in a manner that most people would find reasonable and that does not interfere with other people’s safety or enjoyment. - Avoid conduct which causes, comprises or involved intimidation, harassment or abuse. - Avoid conduct which causes, comprises or involves discrimination, disadvantage or adverse threaten to others. - Report to management any breaches to the ‘Code of Conduct’. - Report any conflict of interest, a professional relationship must be maintained with all clients of the Centre. - Report to the CDO any prescribed or over the counter medications which has the potential to affect your ability to undertake your duties, in particular the operation of machinery. Childsafe/Vulnerable People: - Comply with specific organisational guidelines on physical contact with children. - Respect the privacy of children and their families and only disclose information to people who have a need to know. - Maintain a child safe environment for children, young and vulnerable people. - Treat children and young people with respect, listen to and value their ideas and opinions and protect their wellbeing. - Disclose any existing relationships with children, families or vulnerable people should those person’s start to access services, activities or programmes at the Centre to the Community Development Officer. Access to information and resources: - Protect confidential information - Only access information needed for your role - Not use confidential information for any non-official purpose - Only release confidential information if you have the authority to do so. - Only use confidential information for the purpose it was intended to be used - Only release other information in accordance with established policies and procedures and in compliance with relevant legislation. - Not use information for personal purposes. Centre Resources: - Use centre’s resources ethically, effectively, efficiently and carefully. - Not use resources for private purposes without permission of the Board or the CDO. Employees and Volunteers must not: General: - Contravene the Centres ‘Code of Conduct’ Commit an unlawful act - Assault, or threaten to assault another person present at the Centre or carrying out Centre business - Insult or make personal reflections on or discriminatory comments on another person - Say or do anything that is inconsistent with maintaining good order at the Centre in relation to its facilities, activities, and services, programmes and reputation. Childsafe/Vulnerable People: - Use prejudice, oppressive behaviour or language with children. - Discriminate on the basis of age, gender, race, culture, vulnerability or sexuality. - Initiate unnecessary physical contact with children or do things of a personal nature that children can do for themselves. - Develop ‘special’ relationships with specific children that could be seen as favouritism such as offering of gifts or special treatment. - Engage in rough physical games, including horseplay with children. - Engage in personal or external relationships with children or families who attend the Centre to access services, programmes or activities. Alcohol and Drugs: - Possess, sell, distribute, consume or be under the influence of prohibited drugs whilst the Centre or involved in Centre activities. - Be affected by alcohol or drugs including medically prescribed or over the counter drugs in the sense that your ability to work safely and completely is impaired.

    I Consent to photos taken of me at Christie Downs Community House to be used for promotion and marketing purposes on our WEB page, Brochures and any other marketing material. I release the Christies Down Community House from any claim by me or anyone on my behalf and arising out of my appearance in promotional material. I acknowledge that there is to be no payment or further consideration paid for my photograph.

    All employees, consultants, subcontractors and volunteers working for Christie Downs Community House will uphold the Principles and Objectives of the Disability Services Act (1986), The Privacy Amendment (Private Sector) Act 2000 and the Privacy Policy & Procedure of FWS, and as a consequence, will: - Only collect information that is lawful, necessary and directly related to work - Provide clients and employees of Christie Downs Community House with a guarantee of confidentiality of private/sensitive information - Ensure private information and records are kept secure from loss, theft, unauthorised access or disclosure - Allow freedom of access to private information by the individual concerned - Ensure personal privacy for clients and staff, maintaining dignity and respect at all times - Ensure sensitive conversations are held privately and only with necessary persons - Maintain confidentiality concerning personal employment conditions, including salaries etc. In addition, all organisational documentation, business or financial arrangements, dealings, transactions or affairs and intellectual property, shall remain the property of Christie Downs Community House and shall not be removed, shared or distributed without authorisation of the CDO. I understand that as a condition of me working for Christie Downs Community House that I will maintain confidentiality regarding all client, employee and organisational matters and that I will not divulge any information I have acquired during, or after, my involvement with Christie Downs Community House unless legally required to do so. All confidential records, documents or copies of extracts in my possession will be returned upon termination of my employment. I understand that any breach of confidentiality will render me liable to disciplinary action, termination of employment and/or civil proceedings. I further undertake to inform my manager immediately if I become aware of any breach of privacy relating to the information I access in the course of my duties. I have also read, understood and agree to abide by the Christie Downs Community House Privacy Policy & Procedure.

    We commit to: 1. Ensuring that all volunteers are properly integrated into the Centre structure. 2. Providing each volunteer with the name of a contact point who will look after their interests, provide support and supervision on a regular basis. 3. Assuring that any information shared with the Centre is kept confidential. 4. Assisting volunteers in carrying out their roles by providing induction, training, skills development, information, and support necessary to the role being undertaken. 5. Giving the same protection under health safety regulations and public liability as paid employees. 6. Recognising and valuing volunteers’ contribution. 7. Providing adequate insurance cover for volunteers whilst undertaking duties approved and authorised by the Centre. 8. Endeavouring to resolve any problems, grievances and difficulties which may be encountered while volunteering and provide an opportunity to discuss the issues in an appropriate manner. 9. Including volunteers at relevant meetings and social functions 10. Consulting on matters directly or indirectly affecting voluntary work 11. Ensuring volunteers are not out of pocket, in respect of expenses incurred performing voluntary tasks for the Centre. 12. Issuing written descriptions of the tasks that make up voluntary work. 13. Trusting volunteers with confidential information that is necessary in the course of voluntary work. Volunteers are asked to: 1. Observe the policies, procedures and rules of the Centre as they apply to the activity for which they volunteer. 2. Familiarise themselves with the policies and procedures of the Centre. 3. Be willing to undertake appropriate training with respect to Health & Safety issues, compliance regulations and general good practice that will improve performance in respect of the voluntary work undertaken. 4. Show respect for fellow Centre volunteers, employees, members and customers. 5. Respect and uphold the confidentiality of all information relating to the Centre, its members, employees and customers, during and after cessation of voluntary work. 6. Return immediately on request any document materials or other property which are held on behalf of the Centre 7. Be reliable and punctual and honour any commitment made to the best of ability, such as notifying in good time the relevant member of staff if you are unable to volunteer on a day (such as holidays), or if you are going to be delayed. 8. Carry out your voluntary work as outlined in the agreed description or written guidelines and be prepared to be flexible in order that your voluntary support moves with a growing and changing Centre 9. Contribute to the work of the Centre by attending meetings as necessary 10. If possible, to give two weeks advice of their intention to cease volunteering with the Centre. 11. Recognise the right of the Centre to expect quality of service from all its volunteers 12. Recognise that they represent the Centre and therefore act in an appropriate manner at all times and avoid any word or deed that will bring the Centre into disrepute. Please remember that if a volunteer fails to follow this charter it may damage the Community Centre and its work. The Centre is committed to the principle of equality of opportunity and aims to ensure that all present and potential participants, officials, volunteers and employees are treated fairly and on an equal basis, irrespective of their gender, age, disability, ethnic origin, colour, religion or belief, social status or sexual orientation. The parties commit themselves to use and apply this Charter in the spirit of mutual respect and goodwill and to work together in that spirit, as well as the wording of this Volunteer Charter. This is an enduring agreement to which all parties will be committed. The Charter shall be reviewed as required by agreement of the parties.

    It is important that volunteers and staff maintain proper professional boundaries in regard to participants and community centre users. Therefore, the Centre does not allow the development of personal relationships/friendships or the sharing of personal information between Centre Users/Participants and volunteers and staff. Therefore, if you are working with a Centre User/Participants you need to discuss the following with them so they have a clear understanding of Centre expectations. Staff are not to with volunteer and centre users: - Becoming friends on Facebook - Exchanging gifts. - Exchanging telephone numbers. - Visit each other’s homes. - Share other volunteers/participants information with a third party - Hug or engage in any activity which brings about physical contact of a more intimate nature Volunteers can form personal relationships/friendships with other volunteers. Participants can also form relationships/friendships with other participants. Meaning they can; - Car pool. - Be friends outside of the Centre - Exchange personal information such as phone numbers or become friends on Facebook - Hug and have physical contact I have read the information above regarding professional boundaries and understand my obligations.

    Thank you for offering your services as a volunteer with the Christie Downs Community House. This agreement is to ensure you have a clear understanding of your involvement and responsibilities whilst engaged in a volunteer capacity. I agree to the following conditions: 1. To undertake duties and activities as a volunteer with the Christie Downs Community House of my own free will, without payment (excludes authorised reimbursements) and in accordance with the Christie Downs Community House Code of Conduct (as provided to me for signature). 2. I acknowledge that for insurance purposes I am covered by the Local Government Mutual Liability Scheme Personal Accident Insurance, only whilst I am engaged in activities which are supervised or co-ordinated by Christie Downs Community House staff. 3. To uphold a positive image of the Volunteer Program and the Christie Downs Community House at all times. 4. To perform all volunteer activities, as detailed in my Position Description(s), with due care following established practices, procedures and the instructions of centre staff. Should I be asked to perform any activity that I feel I cannot carry out proficiently or safely, I will discuss this matter with my volunteer co-ordinator or the Community Development Officer. 5. To report any incident, injury, accident, potential hazard, property damage or near miss whilst I am volunteering as soon as practicable to the Community Development Officer. 6. At all times, I will respect any person I associate with in my voluntary role(s) and fully maintain confidentiality and privacy requirements regarding my role(s), including client details and customer information. 7. To participate in training relevant to my volunteer position as required by the Christie Downs Community House. 8. To abide by all associated legislation and Christie Downs Community House guidelines and requirements including, but not limited to; my Position Description(s), Privacy, Work Health & Safety, Equal Opportunities and Child Safe Environments (where appropriate). 9. When required, to fully complete an application for a National Criminal History Check, and/or National Working with Children Check and/or Vulnerable Person Check. I will advise the Community Development Officer of any convictions or allegations of any criminal offence against me that occurs after my check(s) have been completed. 10. I grant the Christie Downs Community House permission to use my likeness, voice and/or words in any media format to promote the activities of the Christie Downs Community House Volunteer Program. 11. I accept that the Christie Downs Community House reserves the right to review my volunteering activity with me and, if deemed necessary to discontinue my volunteering with the Centre. I acknowledge that upon ceasing volunteer activities with the Christie Downs Community House, I have no right to any information contained in e-mail or network file systems of the Christie Downs Community House.
