Registration Statement

I hereby register to participate in 2026 Vacation Bible School and agree that I will adhere to protocols and policies of Community Baptist Church of Englewood (CBC) during all activities. I hereby give consent for CBC to secure and authorize emergency medical treatment for me in the event of an emergency during 2026 Vacation Bible School.

For valuable consideration received, I hereby grant Community Baptist Church of Englewood, its representatives, licensees, and assigns, the right and unrestricted permission to use my photo including, but not limited to use in any and all of its publications, including website entries, without payment or other consideration.

I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless, release and forever discharge Community Baptist Church of Englewood, its representatives, licensees, and assigns from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have but for this authorization.
