    Completing this form does not guarantee receipt of an Play Ready Kit or that kits will have all items requested. 

    Designed to help girls (K - 12th Grade) feel confident, included, and prepared to participate in sports, recreation, outdoor activities, and wellness programs by helping reduce barriers to participation.
