I hereby acknowledge that participation in the Sterling High School Track & Field Feeder Schools Kickball Tournament involves inherent risks of injury.

On behalf of myself and my child, I voluntarily assume all risks and release Sterling High School, its staff, volunteers, organizers, and sponsors from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury or death that may be sustained while participating in tournament activities, whether caused by negligence or otherwise.

I authorize tournament staff to obtain emergency medical treatment for my child if necessary and agree to be responsible for any related costs. I also grant permission for my child to be photographed or videoed during the event and allow the school to use such images for promotional purposes.
