I, the undersigned give permission to my son/daughter(the form applicant) to participate in the above listed sport activities during the current St.Mark Festival session. I realize that there may be an inherent risk of injury. I accept full responsibility for granting permission for my child to participate in the below named sport/s and provide transportation before and after practice sessions and events unless otherwise arranged.

The participant must attend preparatory spiritual lessons at their registered church in order to participate in their sports activity. The passing of the spiritual qualification exam is mandatory to be able to play on the first day of St.Mark's Festival.
