• People of Motueka Photography Competition — Model Consent Form (Child/Minor)

  • This form must be completed by a parent or legal guardian — not by the child/minor.

Thank you for allowing your child to be part of the People of Motueka photography competition, organised by Glasgow Harley. For more information about this competition, visit the competition webpage www.ghlegal.nz/peopleofmotueka and read our competition guidelines here.

This form gives us permission to display the photo(s) your child appears in as part of the competition.

Please read carefully before signing. If you have any questions, contact us at: info@ghlegal.nz or 03 548 7129.

  • SECTION 1 — About the photographer or submitter

This section should be completed by the photographer or the person submitting the photo entry.
  • SECTION 2 — About the photo(s)

Please provide the title(s) of the photo(s) this consent relates to, so we can match it to the correct entry. The photo title(s) in the model form must match the title(s) in the entry form.
  • SECTION 3 — About the child/minor

  • SECTION 4 — About you (parent or guardian)

  • Format: (000) 000-0000.
  • SECTION 5 — Consent

  • By signing this form, I confirm that:

    • I am the parent or legal guardian of the child named above.
    • The child appears as part of a group or family scene — not as the sole or main subject of the photo(s).
    • I give Glasgow Harley permission to display this/these photo(s) for the following purposes:
      • The People of Motueka online gallery and public voting
      • Physical exhibition at Motueka Library (approximately 2 weeks)
      • The People of Motueka art trail across selected Motueka and larger district venues
      • Permanent display at Glasgow Harley's Motueka office
      • Glasgow Harley’s website, social media, newsletter, and marketing communications related to the competition and Glasgow Harley’s community initiatives
      • Press and media coverage related to the People of Motueka competition and Glasgow Harley's community initiatives.
    • I understand that Glasgow Harley will review this/these photo(s) before displaying it publicly and is satisfied that the child's inclusion is appropriate and natural, and does not unnecessarily reveal sensitive personal information such as their school, address, or other identifying details.
    • I understand that once a photo is displayed publicly — online or in print — complete removal from all platforms may not always be possible.
    • I can withdraw my consent at any time by contacting Glasgow Harley at info@ghlegal.nz or 03 548 7129. Glasgow Harley will make every reasonable effort to remove the image from future use.
    • Glasgow Harley will store all images securely and will not use them for any purpose beyond those listed above.
    • Glasgow Harley will keep images only for as long as they are needed for the purposes listed above. Some selected works may be displayed for an extended or indefinite period as part of the exhibition, the art trail, or the office display, and those images will be kept for as long as they remain on display. You can ask us to stop using your child’s image at any time, and we will take reasonable steps to do so.
    • I have explained to the child (in age-appropriate terms) that their photo will be displayed publicly, and they are comfortable with this.
    • When your child turns 18, they, or you on their behalf, can ask us to remove their image from our own displays and channels, and we will take reasonable steps to do so.
    • I understand this/these photo(s) may be reviewed by an independent judging panel. All judges are bound by a confidentiality agreement and may not copy, save, or share any photos they review.
