ASSUMPTION OF RISK I, the undersigned parent or legal guardian, understand that participation in baseball activities involves inherent risks, including being struck by baseballs, bats, or equipment; sliding, diving, running, throwing, and other athletic activities; collisions with players, coaches, spectators, fences, dugouts, or field equipment; weather-related conditions including heat, sun exposure, smoke, wind, and uneven field conditions; and sprains, strains, fractures, concussions, serious bodily injury, permanent disability, or death. I knowingly and voluntarily assume all risks, known and unknown, associated with my child's participation in this camp.
RELEASE OF LIABILITY In consideration of my child's participation, I release, waive, discharge, and covenant not to sue the Tonasket Youth Baseball Association, its officers, directors, volunteers, coaches, instructors, sponsors, camp staff, athletes, landowners, facilities, and affiliated organizations from any and all claims, demands, damages, causes of action, liabilities, costs, or expenses arising out of or related to any injury, illness, loss, damage, or death that may occur during participation in camp activities, whether caused by negligence or otherwise, to the fullest extent permitted by Washington law.
MEDICAL AUTHORIZATION I certify that my child is physically capable of participating in baseball activities. In the event of illness, injury, or emergency, I authorize camp personnel to obtain medical treatment deemed necessary for my child, including transportation to a medical facility. I understand that I am financially responsible for any medical expenses incurred.
PHOTOGRAPHY & MEDIA RELEASE I grant permission for the Tonasket Youth Baseball Association to photograph, film, and use images or video of my child for promotional, educational, fundraising, website, social media, and advertising purposes without compensation.
CODE OF CONDUCT I understand that camp staff reserve the right to remove any participant whose behavior is unsafe, disruptive, abusive, or detrimental to the camp environment. No refund shall be required if removal occurs due to misconduct.
ACKNOWLEDGMENT I HAVE CAREFULLY READ THIS LIABILITY WAIVER, ASSUMPTION OF RISK, AND MEDICAL RELEASE. I FULLY UNDERSTAND ITS CONTENTS AND SIGN IT FREELY AND VOLUNTARILY ON BEHALF OF MYSELF AND MY MINOR CHILD.
