Permissions and Acknowledgement
I understand that OwlQuest may host special events where photographers, videographers, or media representatives may be present. My child may be photographed, filmed, or interviewed during participation. These materials may be used by OwlQuest for promotional and program-related purposes (e.g., website, social media, flyers).

My child will be allowed to walk home on their own and will not be escorted by any staff member. I understand that he/she will be dismissed from the program at 4:45pm. I will release OwlQuest personnel, staff, board and volunteers from any and all liability in connection with having permission to walk home by himself/herself.

Emergency Medical Consent: If my child requires emergency medical care and I cannot be reached, I give OwlQuest permission to obtain necessary medical treatment for my child. I understand that every effort will be made to contact me before and after care is provided. I agree to be responsible for any costs associated with emergency medical treatment my child receives.
OwlQuest follows the 2026–2027 NYC DOE School Calendar and will be closed on days when NYC DOE schools are closed or when students are not in attendance. OwlQuest will also be closed on scheduled half-daysI understand and acknowledge that OwlQuest will not operate on NYC DOE school closures, non-student attendance days, and half-days.
Early Pick-Up from OwlQuest will end at 4:25pm. Students will be cleaning up between 4:25pm- 4:35pm.
All tuition, deposits and registration fees are nonrefundable.

By enrolling my child in OwlQuest, I understand and agree to the following:
- Enrollment is for the full school year (September–June). OwlQuest is not a drop-in or month-to-month program. Under special circumstances, month-to-month enrollment may be approved at OwlQuest’s discretion and will be charged at a rate of $450 per month.
- Late payments may result in a $50 late fee. Tuition will not be reduced, credited, or refunded due to student absences.
- I understand that OwlQuest is a fee-based program and offers flexible dismissal. I may pick up my child early when needed; however, early pickup ends at 4:25 PM to minimize disruption during academic enrichment and program activities.
- My child is expected to follow respectful and school-appropriate behavior expectations. Disruptive, unsafe, or repeated inappropriate behavior may result in verbal reminders, written warnings, parent notification, and possible removal from the program.
- I understand OwlQuest provides dedicated homework support time each day. Staff will make every effort to assist students with completing assignments; however, homework completion cannot be guaranteed, especially if students attend other activities, arrive late, or are picked up early. Parents/guardians remain responsible for reviewing homework at home.
- I will notify OwlQuest in writing via email or text of any changes regarding my child’s contact information, health information, emergency contacts, or authorized pickup information.
- I understand program communication may occur through email, phone calls, or text messages for important updates, reminders, and program-related information.
- I understand participation in OwlQuest activities may involve general risks associated with childcare, recreation, enrichment, and physical activities. I agree to release and hold harmless OwlQuest, Inc., including its directors, officers, employees, instructors, volunteers, and representatives, from claims, liabilities, damages, or expenses related to my child’s participation in the program, except in cases of gross negligence or willful misconduct.
- By submitting this registration form, I acknowledge that I have read, understand, and agree to follow OwlQuest program policies.
- The regular annual program tuition is $3,725. Families who complete full annual payment by the early registration deadline may qualify for the discounted rate. If I choose an installment plan, I understand payments are portions of the full school year tuition and are not based on monthly attendance, holidays, school closures, or the number of program days in a specific month. All final balances due by September 1st. Late fee $50 will apply if not paid on time for installment plans.
I have read, understand, and agree to all OwlQuest policies, terms, and conditions outlined in this registration form, including the Parent/Guardian Program Agreement. I acknowledge that my agreement and consent are effective as of the date this form is submitted and will remain in effect for the duration of my child’s enrollment in OwlQuest.
