    PARTICIPANT WAIVER & RELEASE AGREEMENT

     

    This waiver applies to all Summer Kickoff 2026 classes, tumbling sessions, fitness activities, workshops, and related activities conducted by Poised Perfections Dance Company LLC.


    I, the undersigned, acknowledge that participation in dance, tumbling, fitness, yoga, and related physical activities involves inherent risks, including but not limited to injury, illness, physical strain, falls, collisions, and other unforeseen risks. I certify that I (or my child) am in good physical health and able to safely participate in the selected activities.


    I understand and voluntarily accept all risks associated with participation in dance classes, tumbling sessions, fitness activities, workshops, events, and related activities conducted by Poised Perfections Dance Company LLC.


    In consideration of being allowed to participate, I hereby release, waive, and discharge Poised Perfections Dance Company LLC, its owners, instructors, staff, volunteers, affiliates, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, damage, or accident that may occur as a result of participation.


    I agree to disclose any medical conditions, physical limitations, injuries, allergies, or disabilities that may affect participation. I understand that failure to disclose such information may increase risk and release the organization from responsibility related to undisclosed conditions.


    I acknowledge that Poised Perfections Dance Company LLC is not responsible for any personal property that is lost, damaged, or stolen during participation.


    By signing below, I confirm that I have read, understand, and agree to the terms of this Participant Waiver & Release Agreement. I understand that this agreement applies to the activities selected through this Summer Kickoff 2026 registration.
