Our Commitment: At TAL: The Abilities Lounge, we strive to create a safe, inclusive, and supportive environment where every individual is respected, encouraged, and empowered to express themselves. Behavior Expectations: To ensure a positive experience for all participants: Participants must treat others, staff, and the space and equipment with respect and kindness. No bullying, aggressive, harmful, or unsafe behaviors (physical or verbal). Personal belongings and TAL equipment must be used appropriately at all times. Participants should follow staff guidance at all times for safety. TAL reserves the right to refuse or discontinue services at any time if a participant’s behavior poses a safety risk to themselves, others, or the environment, or if policies are not being followed. Supervision & Support: TAL is a non-medical recreational space. Parents/guardians are required to remain on-site and responsible for their child at all times. Participants who require one-on-one (1:1) assistance must be accompanied by a parent, guardian, or designated caregiver who will actively assist them with engagement in activities as needed.
Photo & Media Release

I give permission for TAL: The Abilities Lounge to take photographs and/or video recordings of the participant(s) listed above during activities, sessions, and events. I understand that these images and recordings may be used for promotional, marketing, and educational purposes, including but not limited to social media, website content, printed materials, and advertisements. I understand that: No personal identifying information (such as full name, medical information, or address) will be shared without additional consent. Images will be used respectfully and in alignment with TAL’s mission of inclusion and empowerment. Families are welcome to take photos or videos of their own child(ren); however, capturing images or recordings of other participants is not permitted without permission from their parent or guardian. I may withdraw this consent at any time in writing.

By signing this form, I agree to assume responsibility for the participant while engaging in activities and release TAL, its owners, and staff from liability for any injuries or incidents that may occur during participation.This facility uses video surveillance for safety and security.
