In consideration for my being permitted to participate in the activities of Camp Sycamore Creek,I/we agree to the following Waiver and Release: In consideration for being permitted to participate in activities of Camp Sycamore Creek, I/we agree to the following Waiver and Release. I/we acknowledge that Camp Sycamore Creek has inherent risk, hazards and dangers for anyone that cannot be eliminated particularly ina Camp environment. My/our participation in this activity is purely voluntary. No one is forcing me/us to participate and I/we elect to participate in spite of the risks.
I/WE AM/ARE VOLUNTARILY USING THE SERVICE OF CAMP SYCAMORE CREEK WITH FULL KNOWLEDGE OF THE INHERENT RISKS, HAZARDS AND DANGERS INVOLVED AND HEREBY ASSUME AND ACCEPT ANY AND ALL RISKS OF INJURY, PARALYSIS OR DEATH. Lastly, I/we, for myself/ourselves, my/our heirs, successors, executors and subrogates, herebyKNOWINGLY AND INTENTIONALLY WAIVE AND RELEASE, INDEMNIFY AND HOLD HARMLESS Camp Sycamore Creek, their directors, officers, agents, employees, and volunteers from and against any and all claims, actions, causes of action, liabilities, suits,expenses (including reasonable attorney's fees) which are related to or arise out of or are in anyway connected with my/our participation in this activity including, but not limited to,NEGLIGENCE of any kind or nature, whether foreseen or unforeseen, arising directly orindirectly out of any damage, loss, injury, paralysis, or death to me/us or my/our property as aresult of my/our engaging in these activities or the use of these services, animals or equipment,whether such damage, loss, injury, paralysis or death results from negligence of Camp SycamoreCreek or from some other cause. I/we for myself/ourselves my/our heirs, successors, executorsand subrogees, further agree not to sue Camp Sycamore Creek as a result of any injury, paralysisor death suffered in connection with my/our use and participation in the activities of CampSycamore Creek.

I understand the zip line, giant swing, leap of faith and/or challenge course participationinvolves inherent risk and participation may result in emotional or physical injury, illness,permanent disability, or death. My participation is completely voluntary. I acknowledge that Ihave had the opportunity to inquire about the Camp Sycamore Creek activities in which I planto participate and have asked any questions i deemed appropriate regarding those activitiesand the risks involved. I assume all risk that could result from any of these activities. I foreverrelease and discharge Camp Sycamore Creek., its staff members, principles, and board, fromany and all claims for any injuries, damages, or loss to me as a result of my participation in anyCamp Sycamore Creek activity. By participating in the program, I agree for any likeness to beused for any commercial purposes.
