Foster Application

Please read the following statements about the foster care program. Initial each box indicating that you understand and agree to abide by these policies and guidelines. Johnson County Animal Shelter henceforth will be abbreviated JCAS.

I understand that JCAS is the sole and rightful owner of animal(s) in foster care. All decisions regarding foster pets are to be made by JCAS staff. I also understand that I do not have the authority to give, transfer, or adopt out custody of any foster animal(s) to any other person or organization at any time. I understand that fostering a pet from JCAS does not guarantee that I will be able to permanently adopt the animal.

I agree to keep JCAS Staff updated on the health, behavior, and temperament of my foster pet(s) and agree to report any medical problem(s) to JCAS immediately. I understand that I must consult with JCAS staff before taking my foster pet(s) to any veterinary practice and no medical procedure can be done without prior authorization from JCAS staff.

I agree to return my foster pet(s) within 24 hours of request by JCAS staff unless other arrangements are made.

I agree that animals in my care will be provided with reasonable exercise (for a dog in a fenced yard or on a leashed walk) and they will be under the immediate control and protection of myself at all times. They will not be permitted to roam freely outside of a fenced area and understand that chaining and tethering are not acceptable forms of confinement.

I agree to incorporate the foster animal(s) into my daily routine and to practice normal household behaviors, such as using a crate, using a leash, encouraging acceptable bathroom habits, giving medication (as directed by the shelter), and continued socialization through appropriate and positive interactions.

I understand that I may have to work with and follow the recommendations of the JCAS staff on behavioral issues the foster pet may have, including but not limited to chewing, basic manners, anxiety, boredom, and any other issues that may arise. I understand that JCAS is not responsible for any damages that may be caused by these behaviors.

I understand that my foster pet may not be house or litter trained, which may result in accidents in my home. Punishment for these accidents is unacceptable.

I understand that if any foster pet is harmed due to my negligence, I will be held responsible for the veterinary bills, surgery costs, and/or medications, and or subject to criminal charges.

I understand that in the unlikely event that a bite, scratch, or injury inflicted by the foster pet(s) results in blood being drawn, I must report the incident to the JCAS Staff immediately in compliance with state regulations.

I agree to supervise any and all interactions that occur between my foster pet(s) and my personal pets, as well as any and all interactions that occur between my foster pet(s) and any children, regardless of whether the children live in my home or not.

I understand that JCAS recommends that foster pet(s) be kept separate from my current pets, for at least 10-14 days for health reasons. In addition, I understand there are some risks associated with bringing a foster pet into my home and JCAS is not responsible for the medical treatment of my personal pets if they get sick or injured as a result of my fostering.

I agree to feed my foster pet(s) separately from any other pets living in the residence. 

I acknowledge that JCAS or any of its representatives have not made any warranties regarding the future conditions, temperament, or conduct of my foster pet(s). I agree and understand that an animal can be unpredictable and that JCAS cannot anticipate or insure against unexpected conduct.

The undersigned, in acting as a volunteer for the Johnson County Animal Shelter, hereby releases the Johnson County Animal Shelter, its agents, officers, servants, and employees of and from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or relating to any losses, damages, or injuries that may be sustained by the undersigned or any of the property undersigned.

The undersigned further agrees to indemnify and save harmless the Johnson County Animal Shelter, its agents, officers, servants, and employees from any and all liability which may hereafter be brought against the Johnson County Animal Shelter by or on behalf of the undersigned for the undersigned’s named minor of any of the foregoing matters hereby released.

The undersigned hereby acknowledges the risk inherent in the handling of animals, domesticated or wild, and hereby willingly accepts all such risks
