THE CHURCH OR THE PASTOR IN CHARGE RESERVES THE RIGHT TO CANCEL OR CHANGE THIS FIELD TRIP/ACTIVITY AT ANY TIME; AND SHALL NOT BE RESPONSIBLE FOR ANY FINANCIAL LOSSES ARISING FROM SUCH CANCELLATION OR CHANGES FOR THE SAFETY OF THE PARTICIPATING CHILDREN, CHAPERONES, VOLUNTEERS AND PASTORS.

As parent/guardian(s) of my Child(ren), I/we agree to take the following responsibilities:
- To provide full and detailed medical, food and medicine information of my child, if participation requires my child to be physically present with other students, Church staff, and/or volunteers for any portion of the field trip/activity;
- To provide full and detail disclosure of any personal health or medical limitations which may place the other students and/or other Church staff and volunteers at risk, if participation requires my child to be physically present with other students, Church staff, and/or volunteers for any portion of the field trip/activity;
- To instruct my child to comply with all established guidelines;
- To instruct my child to obey the instructions of Pastors-in-charge, chaperones, and other leaders;
- To respect facility properties and the properties of others;
- To respect pastors, chaperones, drivers, and all other Church volunteers; and
- To speak with Pastor-in-Charge, chaperone or other volunteer leaders IMMEDIATELY if and when he/she does not feel well, is injured, feels uncomfortable or unsafe.
I/We grant consent for my/our child to attend the Field Trip/Activity being organized by the Church. I/We agree not to hold the Church, its staff, volunteers from any and all personal liabilities for any injury, loss, or damage to person or property that may occur during the course of my child’s participation of the Field Trip/Activity, if and when such injury, loss or damage occurs without the negligence of the involved Church volunteers, including Pastors. In the event that my child is injured and requires the attention of a medical professional, I/we consent to any reasonable medical treatment as deemed necessary by a licensed medical service provider. In the event such medical treatment is required, I/we agree to hold such person free and harmless of any personal liabilities, claims, demands, or suits for damages arising from such decision. I/We acknowledge that we will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance provider. I/we recognize that the Church or the Pastor-in-Charge cannot be held personally responsible for conditions beyond their reasonable control or my child’s breach of his/her responsibilities.
I/We hereby grant to Bethel Korean Presbyterian Church, Inc. (“Bethel Church”) permission and rights to record, play, perform, publish, and/or use recording, singing, performance, and portraits or pictures of my child, in whole or part (“Recording”) in any form or forum, including in CD, DVD, on Website, and in whatever electronic and/or digital media, and without further compensation, for Bethel Church’s Christian Education ministries, including Bethel Youth Group's ("BYG") fall and winter retreats. I/We further authorize, assign, transfer and convey any and all copyright to said recording to Bethel Church.
