Consent and Authorization:
I, the undersigned, am the parent/guardian of the student named above. I hereby grant permission for my child to participate in the upcoming school trip to Bolera de Cupey scheduled to take place June 19, 2026 at 9:30am.
Health Information: I understand that Bailefusión staff will take necessary precautions to ensure the safety and well-being of the students during the trip. I affirm that I have disclosed any medical conditions, allergies, or special health considerations that could affect my child’s participation in this trip.
Emergency Medical Treatment: In the event of an emergency, I authorize the staff to seek medical treatment for my child. I understand that every effort will be made to contact me in case of a medical emergency.
Supervision and Behavior: I acknowledge that my child will be under the supervision of Bailefusión staff and teachers during the trip. I expect my child to adhere to the school's code of conduct and behavioral expectations.
Communication During the Trip: I will provide reachable contact numbers during the trip, and I understand that the school staff may contact me in case of any important updates or emergencies.
Transportation: I acknowledge that transportation to and from the destination will be arranged by bailefusión. I understand that transportation for this field trip will be provided by a licensed and insured third-party transportation company contracted by Bailefusión. I acknowledge that Bailefusión has taken reasonable steps to ensure the safety and reliability of this service provider. I understand that my child is required to travel with the group and follow the designated schedule.
Cost and Payment: I understand the cost associated with the trip $30.00, which includes lunch, 2hrs of bowling and transportation. I agree to make the necessary payment by June 12, 2026.
Dress Code: I agree to comply with the designated dress code for the day of the trip:
Bailefusión t-shirt and comfortable athletic shoes. No sandals, flip-flops, or open-toed shoes are allowed.
Release of Liability: By signing this form, I release and hold harmless Bailefusión, its directors, staff, and affiliated personnel from any and all liability for injuries, accidents, losses, or damages that may arise during or as a result of this trip. This release applies under the laws and jurisdiction of the Commonwealth of Puerto Rico.
I understand that participation in this trip is voluntary and that Bailefusión reserves the right to cancel or modify the excursion due to weather conditions, safety concerns, or other unforeseen circumstances.
