Assumption of Risk and Liability Release: I acknowledge that participation in soccer training, athletic activities, conditioning, matches, and other academy events carries inherent risks of injury, including serious injury or death. I voluntarily assume these risks and release SwitchedOn Saints Academy, its directors, officers, employees, coaches, contractors, and volunteers from liability for any injury, accident, illness, or loss arising from participation in academy activities, except in cases of proven negligence.
Submission of this application does not guarantee acceptance.
If accepted, parents agree to follow the academy tuition payment schedule and policies.
