Camp Survival VBS Participant Agreement: By selecting "I Agree" and registering my child for Camp Survival VBS at Compassion at Calvary, I acknowledge that my child will participate in activities that may involve physical activity, games, crafts, outdoor events, and other camp-related experiences. I certify that my child is physically able to participate and that all medical information provided is accurate. In the event of an accident, injury, illness, or medical emergency, I authorize Compassion at Calvary, its staff, volunteers, and representatives to secure emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any resulting medical expenses.I further grant permission to Compassion at Calvary to photograph, videotape, and record my child during Camp Survival VBS activities and to use such images, videos, and recordings for ministry, promotional, educational, social media, website, live stream, and church communication purposes without compensation.I understand that Compassion at Calvary is not responsible for any lost, stolen, or damaged personal property brought to Camp Survival VBS and that participants are encouraged not to bring valuable items. I voluntarily assume the risks associated with participation and release, waive, and hold harmless Compassion at Calvary, its pastors, staff, volunteers, and representatives from any claims, liabilities, damages, or expenses arising from my child's participation, except in cases of gross negligence or willful misconduct.
