    Uniform Order Form SY‎     2026-27

    Please make sure to fill in the blanks and click submit at the end of this form to complete your order. Parents need to fill out one form for each child enrolled at the Ateneo Grade School. The processing of all personal data collected via this form shall be in accordance with the Privacy Policies of the University. The use of the collected information shall primarily be for processing of services and transactions within A Shop.

    White Polo Shirt 
      distributed by the school
    AGS Khaki Pants
      distributed by the school, but can be purchased outside (strictly observe correct fabric, color, and design)

     

    PE Uniforms (used 1-2x a week)

      distributed by the school
    PE Jogging Pants
      distributed by the school, may use previous schoolyear's

    Girl's Gala Blouse
        distributed by the school. used for Holy Communion, school-wide masses. For Grades 2 and up only.
    Girl's Gala Skirt
        distributed by the school. used for Holy Communion, school-wide masses. For Grades 2 and up only.
    Boy's Gala Barong
        distributed by the school. used for Holy Communion, school-wide masses. For Grades 2 and up only.

    *If the student’s measurements exceed the standard size, the made-to-order (MTO) measuring may be done during the AGS Walk Through Schedule. Otherwise please wait for the next available schedule for measurement from A Shop through the AGS Admin.

  • White Polo Shirt
    The White Polo Shirt is the regular or daily Ateneo uniform. It has the Ateneo seal embroidered on the left chest. It will have a grade level patch for K to 6 and will also be the uniform until G10. The student may opt to continue using last year's (SY25-26) polo shirt however the patch needs to be change/updated.

  • AGS PE Shirt
    AGS PE Shirts are required for all Ateneo Grade School students. The color will depend on the section. Each shirt is customized with the students' class number and start of production is highly dependent on the generation of the final class lists. Because of this, the distribution of the PE shirts will begin in September. You may wish to order 1 to 2 sizes bigger to give allowance for the growth of your child.

    Request for changes in the orders reflected in the Jot Form (e.g. size, quantity) after payment may only be accommodated the following week after your purchase as this will need to be done via return/exchange at the POS.

    Only uniforms with factory defects (e.g. torn fabric, fabric with holes, misprint, etc.) can be replaced; uniforms that need alterations to perfectly fit the students' body will not be accommodated for replacement.
