  • Thank you for your interest in the WSCC School Health NJ Project. This form helps us get to know your school before we meet. It should take about 5-10 minutes to complete. A program coordinator will follow up within 5-7 business days. The WSCC School Health NJ Project for Northern NJ provides schools with the resources, technical support, and programming to build healthier school communities — without requiring schools to do it alone. We connect your team to evidence-based programming, community partnerships, and tools tailored to your school's needs and capacity. Participating schools receive tiered annual funding, subject to federal fund availability, to help bring their health and wellness goals to life. We also facilitate a regional network of participating schools, creating opportunities for shared learning and collaboration across districts that rarely have the chance to connect. Our role is to strengthen and amplify the health and wellness work your school is already doing — for students, staff, families, and the broader school community. The school must be located in one of the 7 Northern NJ counties of Warren, Sussex, Morris, Passaic, Hudson, Bergen, or Essex. 

  • Eligibility: This program serves schools with students in 8th grade or above. Elementary schools that include an 8th grade are eligible. Schools serving only grades K-7 do not meet current program requirements.

  • The Health & Wellness Leader is the driving force behind this program at your school — a natural connector who can be any staff member with the passion, follow-through, and genuine commitment to see their school community thrive. They seek opportunities, rally people around a shared goal, and bring energy to the health and wellness work already happening in the building. 

    In this role, the HW Leader attends monthly virtual check-ins with CFPC staff, coordinates with School Health Coordinators, leads the school's Health & Wellness Team, and submits required program updates. This role requires approximately 8-9 hours per month during the school year (~100 hours annually).
    Selected schools receive an annual program stipend, a portion of which is designated specifically for the Health & Wellness Leader in recognition of their commitment to this work.

  • After reviewing your submission, we'll reach out to schedule an informal introductory meeting — a chance to learn more about each other and answer your questions before any commitment is made. We're heading into summer and are happy to meet in June, July, or August if that works for you.

  • Selected schools will be expected to:
  • Work toward building a healthier school community using the WSCC model as a framework for whole-school change.
  • Designate a Health & Wellness Leader to serve as the primary grant contact at your school.
  • Complete required grant documentation on a quarterly basis — progress updates, attendance tracking, and partnership reporting.
  • Engage in at least one school health improvement effort each grant year — wellness events, equipment, environment improvements, community partnerships, or other school-identified priorities.
  • Participate in Social-Emotional Learning (SEL) programming with students in 8th grade and above. SEL is a required component of this grant. CFPC provides full support, training, and materials.
  • Support student leadership through the Youth Advisory Board (YAB), engaging students in 8th grade and above.
  • Work toward sustaining program gains beyond the grant period by embedding health and wellness into your school's culture and community.
  • This is a tiered, multi-year program. Schools are reviewed annually and may continue for up to 4 years based on participation and federal fund availability. Submission of this form does not guarantee selection. Selected schools will receive a formal grant agreement.
