• All participants of rentals are required to sign below Waiver in advance. Guests under 18 must have a parent sign a waiver prior to participating.
• Rental groups will be allowed to enter the building 10 minutes before their scheduled time.

• Rental are limited to 20-25 players
• Rental-only Volleyballs allowed in Gym, NO OUTSIDE VOLLEYBALLS ALLOWED.
• Rentals must be paid at the time of rental.
• Rentals must leave court by ending time, failure to do so will result in pro-rated payment for extra time used.
• Rental Rates are for individual activities or practice times for teams. It is prohibited to rent courts to tournaments or leagues without the specific permission of management.
• (water bottles allowed). No Food. No Alcoholic Beverages. No Smoking and No Pets allowed.
• The user agrees to leave the premises in as good or better condition than which existed prior to their usage. All trash must be disposed of properly.Failure to do so will result in a $50 cleaning fee charge.

alone in the facility.

• No one is allowed upstairs, in the director's office, kitchen or other rooms.
• Failure to comply WILL result in removal of the player or entire group from premises.

• Spectators are allowed.
• Spectators are not permitted to use workout equipment or roam the facility. They are only permitted in the spectator seating area inside of the gym.

• All times shall be rented on the hour or half hour, in one hour or half hour long increments.

• The renter agrees to leave all rented areas dean and in good order. User shall also be responsible for any extraordinary deaning costs or damage to the building or equipment by the using group. For groups making regular use of the Boltz Gym facility, the usage fee may, at the discretion of the Executive Director or his/her designee, be adjusted upward to cover such costs. The Boltz Gym assumes no responsibility for property brought into the building Renter agrees Keep the gym area dean (All Volleyballs back into the pins and NO water bottles and Trash on the floor), the $50 deaning fee may be applied to cover extra staff expenses for cleaning.

Doors must be dosed and locked and all lights must be turned off when you leave unless another approved coach is in the gym before you leave. Players nor parents will be left in the gym without an approved coach. If you are the last practice of the night you are responsible to dose the gate to the parking lot as well.

• I acknowledge the receipt of the code to the lock box and code to the parking lot gate.

• I acknowledge that I am only granted access to the facility during my team(s) scheduled practice time and pre-approved scheduled times. Unauthorized access may result in termination of my involvement with the Boltz Organization.
• Keys must be returned to the lock box immediately after opening the door. Keeping or duplicating the key(s) or giving out any combination lock/codes given is strictly prohibited. The code shall be used by contract signers only.
• If the key(s) for the above mentioned property are not returned to the box immediately after the door is opened, or leaves the property with you or the codes are shared the assigned holder hereby agrees to pay a $300 fee for re-keying/ and coding fee. These fees are due 15 days from the original reservation date.
• Lost or stolen key(s) shall be reported as soon as they are noticed as missing.

• All Reservation payments, including deposits and Account Set up fees, are non-refundable. If the facility is closed for any reason, customers shall receive a facility credit toward a future rental, date. No refunds will be given. 

Boltz Athletics & Fitness priority is with our members. We reserve the right to cancel or extinguish a contract at any time without prior notice. I understand that I am responsible for my group and that I am responsible for any and all damage to the gym, building or equipment. I have read and fully understand the above statements.
