Download the Endorsement Form: Before submitting, please download the endorsement form PDF. You and your potential member must review and complete this form together.

Check Your Inbox: Within 5 minutes of submission, you will receive an email containing your queue number ticket.

Submission: Once you have completed the form, please email it to elevatefvdm@gmail.com using the following subject line format: POTENTIAL MEMBER'S FIRST NAME_LAST NAME_ENDORSEMENT FORM

Confirm the Schedule: You will receive a separate email regarding your potential member's interview schedule. Please reply to that email to confirm his/her availability.
