I understand that this information will be relied upon by Oakland American School in making enrollment and grade placement decisions.I acknowledge that Oakland American School is a California-registered private school, fully accredited by the Accrediting Commission for Schools, Western Association of Schools and Colleges (ACS WASC), and recognized as a Cognia Member Institution.I agree to provide all required supporting documents in a timely manner and to promptly notify the school of any changes to the information submitted in this application, including changes to contact details, residency, or family circumstances.I consent to the collection and use of my child's information by Oakland American School for purposes of enrollment, academic placement, progress reporting, accreditation review, and administrative operations, in accordance with the school's policies.I further understand and accept that enrollment is not final until completion of academic review, grade placement evaluation, and verification of all submitted documentation by the school administration.

I confirm that all documents submitted in support of this application — including report cards, transcripts, certificates, and identification documents — are authentic, unaltered, and complete. I understand that the submission of false, forged, or altered documents constitutes grounds for denial of enrollment, withdrawal of an enrollment offer, or dismissal from the school, at any stage.I acknowledge that Oakland American School may review the equivalency of academic credentials issued by previous schools or educational systems, and that grade placement is determined by the school based on this review.I authorize Oakland American School to contact and request academic records, transcripts, and related information from my child's previous schools and educational institutions as needed for enrollment and placement purposes.
Financial Obligations Acknowledgments

I agree to pay all tuition and fees as communicated by Oakland American School, according to the payment schedule provided at the time of enrollment. I understand that tuition and fee amounts are set by the school and are subject to the terms communicated in official school documents.I confirm that I have been informed of, and understand, the school's refund policy, and I acknowledge that all withdrawal and refund matters are governed exclusively by that policy.I further acknowledge that all financial obligations to the school remain in full effect until they are formally settled or resolved with the school administration, and that the school may withhold official documents, including transcripts and certificates, until outstanding balances are cleared, to the extent permitted by applicable law.
School Policies Acknowledgments

I agree that my child and I will comply with all policies, procedures, and regulations of Oakland American School, including those set forth in the Student & Family Handbook, as updated from time to time.I acknowledge that the Student & Family Handbook has been made available to me for review, and that it is my responsibility to read it and to direct any questions to the school administration.I understand that the school may require placement or diagnostic testing to determine the appropriate grade level or academic program for my child, and that the results of such testing are determined solely by the school.
Instruction Delivery Acknowledgments

I acknowledge that Oakland American School delivers live, scheduled, synchronous instruction via Zoom, and that classes follow a fixed timetable communicated by the school.I understand that regular attendance and active participation in all scheduled classes are required, and that attendance is recorded and may affect academic standing, grades, and promotion.I agree to ensure that my child has consistent access to a stable internet connection and a suitable device, and I understand that additional technology requirements (such as a working camera, microphone, or specific software) may apply and are the family's responsibility.
