Private Membership Association Agreement

This Private Membership Association Agreement (“Agreement”) is entered into voluntarily by the undersigned individual (“Member”) and With Cyd (“Association” or “Practitioner”).
By signing this Agreement, the Member affirms that they are seeking private wellness, spiritual, educational, and self-development services within a private membership setting, and not as a member of the general public.
1. Purpose of the Association
With Cyd exists as a private association for individuals seeking holistic support, spiritual connection, nervous system nourishment, intuitive reflection, embodiment practices, and integrative wellness experiences.
Offerings may include, but are not limited to:
Intuitive touch healing
Guided embodiment in real time
Nervous system regulation support
Intuitive reflection
Presence-based wellness sessions
Soul Alignment Sessions (birth chart insight + intuitive guidance)
Clarity Calls
Educational resources and membership offerings
2. Voluntary Private Membership
I understand that by signing this Agreement, I am voluntarily becoming a private member of With Cyd.
I understand that all services, conversations, sessions, and exchanges occur privately between members of this Association.
Membership may be accepted, declined, suspended, or revoked at the discretion of the Association.
3. Nature of Services
I understand that services offered through With Cyd are spiritual, educational, supportive, and wellness-oriented in nature.
These services are intended to support self-awareness, relaxation, clarity, emotional presence, embodiment, and personal growth.
No specific outcomes are promised or guaranteed.
4. No Medical or Mental Health Services
I understand that the Practitioner is not a licensed physician, psychologist, psychotherapist, chiropractor, or massage therapist.
The Practitioner does not diagnose, prescribe for, treat, or cure any disease, disorder, injury, trauma, or medical condition.
Services are not a substitute for licensed medical care, mental health treatment, or emergency services.
I agree to seek appropriate licensed providers for medical, psychological, or emergency concerns.
5. Not Massage Therapy / Touch Disclaimer
I understand that the Practitioner is not a licensed massage therapist.
Any touch offered during sessions is intended for energetic support, grounding, presence, comfort, embodiment awareness, and relaxation only.
Services provided by With Cyd are not massage therapy, medical bodywork, physical therapy, or treatment of soft tissue conditions as defined by applicable law.
I understand I may decline touch, request modification, or stop touch at any time.
6. Consent and Personal Responsibility
I participate voluntarily and accept full responsibility for my choices, interpretations, emotional responses, and personal wellbeing.
I understand that growth-oriented and intuitive work may bring emotional insight, reflection, or discomfort, and I choose to engage responsibly.
7. Payment Policies
Fees, donations, memberships, packages, and session rates are agreed upon in advance.
Payments are due at the time of booking unless otherwise arranged.
Cancellation, rescheduling, and membership terms may be communicated separately and are incorporated into this Agreement.
No refunds are guaranteed after services are rendered, except at the discretion of the Association.
8. Confidentiality
With Cyd values privacy and discretion.
Personal information and session content will be held respectfully and confidentially, except where disclosure is required by law or necessary to prevent imminent harm.
9. Release of Liability
To the fullest extent permitted by law, I release and hold harmless SouLuna Integrative Wellness and its Practitioner from claims arising from my voluntary participation, except in cases of gross negligence or intentional misconduct.
10. Governing Law
This Agreement shall be governed by the laws of the State of Utah.
Any disputes arising from this Agreement shall be addressed in the appropriate jurisdiction within Utah.
11. Member Acknowledgment
By signing below, I acknowledge that:
I have read and understood this Agreement
I am joining voluntarily as a private member
I understand the nature and limits of services offered
I understand the Practitioner is not a licensed massage therapist
I consent to participate freely and responsibly
