Revere Lions Soccer Club Liability Waiver

In exchange for participation in the activity of soccer organized by Chaib soccer and or the use of the property, facilities, and services of the city of Revere, Ma. I agree to observe and obey all posted rules and warnings and further agree to follow any oral instructions or directions given by anyone who represents the organization and or the city of Revere. I recognize that there are certain inherent risks associated with the above-described activity and I assume full responsibility for personal injury to my child, families' use of the facility and sports. I will not hold the organization of Chaib soccer, its workers, nor the city of Revere responsible for injury or property damage. I am solely responsible for my family while on and participating in actives at any facility with Chaib soccer and any workers. This includes but not limited to damage, injury, or loss from damage. At no time will Chaib soccer, employees, or city of Revere be responsible for families who participates in any activity this includes but not limited to attending events, watching, playing, or on the campus for any reason. I agree to pay for all damages to the facilities caused by neglect, recklessness, or willingly actions by me or my family. In the event of an injury during the time at the facilities, I give permission for Chaib soccer and the employees to arrange for medical treatment for which I shall be financially reasonable. By signing this document you are waiving your legal rights to injury trial to hold Chaib soccer and employees legally responsible for any injuries, or damages resulting from risk inherent in the sport or recreational activity or for any injury or damages you or your family may suffer due to the providers ordinary negligence that are the results of the providers failure to exercise responsible care.
