The Hogue Foundation – Lady Owls Girls Summer Basketball Camp

Location: School 23, 56 Van Cortlandt Park Ave, Yonkers, NY 10701

When: July 13 - August 14, 2026

In consideration for my child being permitted to participate in The Hogue Foundation Lady Owls Girls Summer Basketball Camp ("Camp"), I acknowledge and agree as follows:

Assumption of Risk
I understand that participation in basketball and related camp activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, illness, and other risks that may result in serious injury, permanent disability, or death. I voluntarily assume all risks associated with my child's participation in the Camp.

Release of Liability
To the fullest extent permitted by New York law, I, on behalf of myself, my child, and our heirs, executors, administrators, and assigns, release, waive, and discharge The Hogue Foundation, its officers, directors, employees, volunteers, coaches, agents, sponsors, and affiliated organizations from any and all claims, liabilities, demands, damages, costs, or causes of action arising from or related to my child's participation in the Camp, except where caused by gross negligence or willful misconduct.

Medical Authorization
I certify that my child is physically able to participate in camp activities. In the event of an emergency, I authorize The Hogue Foundation and its representatives to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

Indemnification
I agree to indemnify, defend, and hold harmless The Hogue Foundation, its officers, directors, employees, volunteers, coaches, agents, sponsors, and affiliates from any claims, liabilities, damages, costs, or expenses, including reasonable attorney's fees, arising from my child's participation in the Camp or from any acts or omissions of my child during participation.
Media Release - I grant The Hogue Foundation permission to photograph, record, and use my child's name, image, voice, likeness, and participation in photographs, videos, digital media, social media, promotional materials, publications, and websites for educational, promotional, and fundraising purposes without compensation. I understand that these materials may be used indefinitely and become the property of The Hogue Foundation.
Acknowledgment
I have carefully read this Liability Waiver, Assumption of Risk, Indemnification, and Media Release Agreement. I understand its contents and sign it voluntarily on behalf of myself and my child.
