  • Section 2 — SVHE Membership and Community Standing

  • Are you a current SVHE member?*

  • (These questions relate to the Established Adult Coach criteria in the Sports Operations Manual, Section 8.4. This standard is required for both head coaches and assistant coaches in opposite-sex coaching situations (Section 8.7.1). It does not apply to coaches working with a team of the same sex.)

  • Section 3 — Coaching and Sport Experience

  • Have you previously coached or volunteered with SVHE sports?*
  • Section 4 — Family Relationship Disclosure

  • SVHE’s family relationship policy (Sports Governance Policy, Section 8) prohibits coaches from serving in a direct supervisory relationship with a spouse, anyone to whom they are engaged or in a committed romantic relationship with, or anyone they are pursuing a romantic relationship with. Please disclose any such relationships with others in the sports program.

  • Do you have a spouse, anyone to whom you are engaged or in a committed romantic relationship with, or anyone you are pursuing a romantic relationship with, who currently serves in any SVHE sports role (coach, Sport AD, board member, treasurer)?*
  • Do you have a child currently participating as a player in the SVHE sport you are applying to coach?*
  • Note: Coaching your own child is permitted. This question is asked for transparency so the Sport AD is aware of the relationship at the start of the season.

  • Section 5 — Opposite-Sex Coaching Disclosure

  • SVHE’s opposite-sex coaching policy (Sports Operations Manual, Section 8.7) requires coaches working with players of the opposite sex to meet the Established Adult Coach criteria and comply with additional supervision requirements.

  • Are you applying to coach a team whose players are primarily of the opposite sex from you?*
  • If yes, the following apply to your application:

    • You must meet all six Established Adult Coach criteria listed in Section 8.4 of the Sports Operations Manual
    • As head coach of an opposite-sex team, SVHE expects an assistant coach of the same sex as your players to be present at all practices, games, and team events. That assistant coach must themselves meet the Established Adult Coach definition in Section 8.4. If the assistant coach is unavailable for a given practice or event, all activity must be held in a fully open and observable setting consistent with Policy 6.2
    • A head coach may never be alone with a single minor in any private or unobservable setting under any circumstances

    Two-deep supervision is the standard for all SVHE activities; see Policy 6.2 for full details

  • Section 6 — Character and Conduct Screening

  • These questions are required by SVHE’s Child Protection Policy. All answers are reviewed by the Sport AD and Athletic Board. False or incomplete answers are grounds for immediate disqualification.

  • Have you ever been convicted of, charged with, or are you currently under investigation for any criminal offense?*
  • Have you ever been the subject of a complaint, allegation, or investigation involving inappropriate conduct with a minor?*
  • Have you ever been removed from, asked to resign from, or denied a coaching or volunteer position in any youth sports, school, or church program?*
  • Section 7 — Recent Graduate Disclosure

  • SVHE’s recent graduate coaching guidelines (Sports Operations Manual, Section 8.5) apply to former SVHE players seeking a coaching role within one season of their graduation from SVHE sports.

  • Were you a player in SVHE sports within the past two seasons?*
  • Conflict history is a required part of the coach selection process. Before any appointment is confirmed, the head coach and Sport AD will review whether unresolved relational conflict exists between the candidate and any current player. Your honest answer below is part of that review.

  • Are you aware of any unresolved relational conflict between yourself and any current SVHE player in the sport you are applying to coach?*
  • Section 8 — Availability and Commitments

  • SVHE coaching roles are volunteer positions and we understand that our coaches have jobs, families, and other responsibilities. This section helps the Sport AD understand your general availability so we can plan effectively and set realistic expectations together.

  • Schedule flexibility: Does your work schedule generally allow availability for practices and games throughout the season?*
  • Section 9 — References

  • Provide two references who can speak to your character, coaching ability, and suitability to work with youth. References may not be immediate family members. At least one reference should be your pastor or a church elder.
