ACTIVITY PARTICIPATION
I understand that I am voluntarily participating in fitness activities offered by VibraFit, including but not limited to dance fitness classes, stretching, mobility, wellness activities, and group exercise programs.
These activities may take place indoors, outdoors, in classrooms, parks, studios, gyms, rented facilities, or other approved locations.
ASSUMPTION OF RISK
I understand that participation in physical activity involves inherent risks, including but not limited to:
• Muscle soreness
• Strains and sprains
• Slips, trips, and falls
• Joint injuries
• Heat exhaustion
• Dehydration
• Allergic reactions
• Heart-related incidents
• Property damage
• Serious injury
• Permanent disability
• Death
I voluntarily assume all risks associated with participation.
MEDICAL ACKNOWLEDGMENT
I certify that I am physically capable of participating in fitness activities.
I understand it is my responsibility to consult a physician before beginning any exercise program.
I agree to stop exercising immediately if I experience pain, dizziness, shortness of breath, chest pain, or any other concerning symptoms.
RELEASE OF LIABILITY
To the fullest extent permitted by law, I release and hold harmless VibraFit, its owner(s), instructors, assistants, employees, volunteers, contractors, affiliates, landlords, facility owners, and representatives from any and all claims, demands, damages, losses, injuries, expenses, liabilities, or causes of action arising from my participation in any VibraFit activity.
OUTDOOR ACTIVITY ACKNOWLEDGMENT
For outdoor classes, I understand and accept risks associated with:
• Uneven ground
• Grass, dirt, gravel, and pavement
• Weather conditions
• Sun exposure
• Insects and animals
• Public spaces
• Other participants
• Natural hazards
I voluntarily assume all such risks.
PHOTO & VIDEO RELEASE
I grant VibraFit permission to photograph, video record, and use my image, likeness, and participation for marketing, advertising, promotional materials, social media, website content, and other business purposes without compensation.
PERSONAL PROPERTY
I understand VibraFit is not responsible for lost, stolen, or damaged personal property, including phones, wallets, jewelry, fitness trackers, vehicles, or other belongings.
AGREEMENT
By signing below, I acknowledge that:
• I have read this entire agreement.
• I understand the risks involved.
• I voluntarily choose to participate.
• I release VibraFit from liability to the fullest extent permitted by law.
• I understand this agreement remains in effect for all current and future VibraFit activities unless revoked in writing.
