By submitting payment, scheduling a trial appointment, checking an acceptance box, replying "I Agree," or electronically signing this document, you acknowledge and agree to the following:

• I understand that my bridal trial is a consultation and preview appointment intended to develop my wedding-day hair and/or makeup look.
• I understand that final wedding-day results may vary slightly due to weather, lighting, hair texture, skin condition, product performance, and adjustments requested by me.
• I have disclosed any allergies, skin sensitivities, medical conditions, cosmetic procedures, or product reactions that could affect services.
• I voluntarily receive hair and makeup services and understand that reactions to products are possible despite professional care and sanitation practices.
• I understand that approval of my trial look indicates satisfaction with the direction, style, and recommendations discussed during the appointment.
• I grant OnSiteGlam permission to photograph my completed hair and makeup services for portfolio, website, social media, educational, and marketing purposes unless I provide written notice requesting otherwise.
• I understand that a bridal trial does not reserve my wedding date. Wedding dates are secured only after receipt of the required retainer and completion of booking requirements.
• I understand that OnSiteGlam may assign qualified hair and makeup artists based on availability, scheduling needs, and wedding size.
• I release and hold harmless OnSiteGlam, its owners, employees, independent contractors, and artists from claims related to allergic reactions, sensitivities, dissatisfaction resulting from undisclosed preferences or conditions, or circumstances beyond reasonable control.
• I acknowledge that trial fees are non-refundable once services have been performed.

By selecting "I Agree," submitting payment, replying with written acceptance, or electronically signing below, I acknowledge that I have read, understood, and agree to the terms of this Bridal Trial & Consultation Agreement.
