1. Assumption of Risk
I understand that participation in this activity involves inherent risks, including the risk of injury. I voluntarily assume full responsibility for any injury or harm to myself and/or my child that may occur during participation.
2. Release of Liability
I release and hold harmless the host, its owners, employees, and representatives from any claims, liabilities, damages, or expenses arising from participation in this drop-in class, except in cases of gross negligence or willful misconduct.
3. Responsibility for Child
I understand that I am responsible for supervising my child at all times during the class and for their behavior, health and safety. If my child is not yet completing toileting independently, I assure that I will be available to assist my child.
4. Damages
I agree to be financially responsible for any damage caused by myself or my child due to negligent or intentional actions.
5. Medical Authorization
In the event of an emergency, I authorize staff to seek medical assistance, including contacting emergency services if necessary.
6. Photography Notice
I understand that the business may take photographs or video from time to time for general promotional or documentation purposes. I understand that my child may be incidentally included within the frame of such media; however, the business will not intentionally capture or use identifiable images of my child’s face.
